Myoepithelioma- An Uncommon Parotid Tumour in An Adolescent. 肌上皮瘤--青少年中一种不常见的腮腺肿瘤。

Myoepitheliomas are uncommon neoplasms of major and minor salivary glands. These tumours are encapsulated and slow growing in nature and are benign in majority of the cases. The mean age of presentation is 40 years and this entity is rarely reported in younger age groups. Here, we seek to report a rare case of a 10-year-old patient who presented with a swelling below the right ear. Clinical and radiological evaluation revealed a mass in the parotid gland. This patient was treated with a superficial parotidectomy and subsequent histopathological examination revealed it to be a myoepithelioma.

Dedicated Chalazion Clinic as a Tool for Early Surgical Education in Ophthalmology Residency. 将霰粒肿专科门诊作为眼科住院医生早期外科教育的工具。

Objective  Ophthalmology residency programs aim to improve resident surgical teaching through increased surgical exposure over a 4-year period. Resident-centric surgical clinics across various surgical specialties have been established to help develop surgical autonomy and experience. We present the first demonstration of a resident-centric chalazion incision and drainage clinic (chalazion clinic) in an ophthalmology residency with the goal of increasing early surgical exposure to residents. Design  The chalazion clinic was founded in July 2019. It is a once weekly procedure clinic conducted by an ophthalmology resident and supervised by an ophthalmology attending. Patients with chalazia were referred directly to this clinic for evaluation and management, rather than the oculoplastics clinic as they were in the past. Retrospective review of Accreditation Council for Graduate Medical Education (ACGME) case logs of all residents per academic year before and after establishment of the chalazion clinic was performed in order to assess the impact on residents' chalazion procedures numbers per academic year. Setting  The study involved a single academic ophthalmology department. Participants  Ophthalmology residents of all years were present. Results  A resident of any year performed an average of 3.0 chalazion procedures per year in the 2018 to 2019 academic year, 3.8 in 2019 to 2020, and 8.4 in the 2020 to 2021, which represents a 180% increase in procedure numbers per resident. Among post-graduate-year 2s (PGY)2s, the average number of chalazion procedures increased from 2.1 procedure per year to 22.3 per year (961.9% increase). Conclusion  To the best of our knowledge, this is the first description of a dedicated resident-centric chalazion clinic in an ophthalmology residency program. PGY2s demonstrated the largest increase in procedural numbers. While chalazion incision and drainage is a minor procedure, increased exposure to surgical procedures early in training could help improve residents' skills and confidence. This clinic provides a proof of concept of a dedicated minor procedure clinic for ophthalmology residents to increase early procedural volume.

Genetics in Behcet's Disease: An Update Review. 白塞氏病的遗传学:最新回顾。

Behcet's disease (BD) is one of the most vision-threatening clinical entities of uveitis. Although the etiopathogenesis of BD remains obscure, accumulating evidence has demonstrated that both genetic and environmental factors may contribute to the development of BD. Genome-wide association studies (GWAS) and candidate association studies have identified several genetic variants strongly associated with BD, including variants in human leukocyte antigen (HLA) -A02, -A03, -A24, -A26, -A31, -B15, -B27, -B35, -B49, -B51, -B57, -B58, -C0704, CIITA, ERAP1, MICA, IL1A-IL1B, IL10, IL12, IL23R, IL-23R/IL-12RB2, IL1RL1-IL18R1, STAT4, TFCP2L1, TRAF5, TNFAIP3, CCR1/CCR3, RIPK2, ADO-ZNF365-EGR2, KLRC4, LACC1, MEFV, IRF8, FUT2, CEBPB-PTPN1, ZMIZ1, RPS6KA4, IL10RA, SIPA1-FIBP-FOSL1, VAMP1, JRKL/CTCN5, IFNGR1 and miRNA-146a. Epigenetic modifications are also reported to play essential roles in the development of BD, including DNA methylation and histone modification. We review here the recent advances in the genetic and epigenetic factors associated with the BD pathogenesis.

Modelling Genetic Benefits and Financial Costs of Integrating Biobanking into the Captive Management of Koalas. 建立将生物库纳入考拉圈养管理的遗传效益和财务成本模型。

Zoo and wildlife hospital networks are set to become a vital component of Australia's contemporary efforts to conserve the iconic and imperiled koala (Phascolarctos cinereus). Managed breeding programs held across zoo-based networks typically face high economic costs and can be at risk of adverse genetic effects typical of unavoidably small captive colonies. Emerging evidence suggests that biobanking and associated assisted reproductive technologies could address these economic and genetic challenges. We present a modelled scenario, supported by detailed costings, where these technologies are optimized and could be integrated into conservation breeding programs of koalas across the established zoo and wildlife hospital network. Genetic and economic modelling comparing closed captive koala populations suggest that supplementing them with cryopreserved founder sperm using artificial insemination or intracytoplasmic sperm injection could substantially reduce inbreeding, lower the required colony sizes of conservation breeding programs, and greatly reduce program costs. Ambitious genetic retention targets (maintaining 90%, 95% and 99% of source population heterozygosity for 100 years) could be possible within realistic cost frameworks, with output koalas suited for wild release. Integrating biobanking into the zoo and wildlife hospital network presents a cost-effective and financially feasible model for the uptake of these tools due to the technical and research expertise, captive koala colonies, and ex situ facilities that already exist across these networks.

Numerous recent works highlight the limited utility of established tumor cell lines in recapitulating the heterogeneity of tumors in patients. More realistic preclinical cancer models are thought to be provided by transplantable, patient-derived xenografts (PDXs). The inter- and intratumor heterogeneity of PDXs, however, presents several challenges in developing optimal quantitative pipelines to assess response to therapy. The objective of this work was to develop and optimize image metrics for 18F-FDG PET to assess response to combination docetaxel and carboplatin therapy in a co-clinical trial involving triple-negative breast cancer PDXs. We characterized the reproducibility of standardized uptake value (SUV) metrics to assess response to therapy, and we optimized a preclinical PERCIST paradigm to complement clinical standards. Considerations in this effort included variability in tumor growth rate and tumor size, solid tumors versus tumor heterogeneity and a necrotic phenotype, and optimal selection of tumor slices versus whole tumor. Methods: A test-retest protocol was implemented to optimize the reproducibility of 18F-FDG PET SUV thresholds, SUVpeak metrics, and preclinical PERCIST parameters. In assessing response to therapy, 18F-FDG PET imaging was performed at baseline and 4 d after therapy. The reproducibility, accuracy, variability, and performance of imaging metrics to assess response to therapy were determined. We defined an index called the Quantitative Response Assessment Score to integrate parameters of prediction and precision and thus aid in selecting the optimal image metric to assess response to therapy. Results: Our data suggest that a threshold of 25% of SUVmax (SUV25) was highly reproducible (<9% variability). The concordance and reproducibility of preclinical PERCIST were maximized at α = 0.7 and β = 2.8 and exhibited a high correlation with SUV25 measures of tumor uptake, which in turn correlated with the SUV of metabolic tumor. Conclusion: The Quantitative Response Assessment Score favors SUV25 followed by SUVpeak for a sphere with a volume of 14 mm3 (SUVP14) as optimal metrics of response to therapy. Additional studies are warranted to fully characterize the utility of SUV25 and preclinical PERCIST SUVP14 as image metrics for response to therapy across a wide range of therapeutic regimens and PDX models.

More realistic preclinical cancer models are thought to be provided by transplantable, patient-derived xenografts (PDXs). The inter- and intratumor heterogeneity of PDXs, however, presents several challenges in developing optimal quantitative pipelines to assess response to therapy. The objective of this work was to develop and optimize image metrics for <sup>18</sup>F-FDG PET to assess response to combination docetaxel and carboplatin therapy in a co-clinical trial involving triple-negative breast cancer PDXs. We characterized the reproducibility of standardized uptake value (SUV) metrics to assess response to therapy, and we optimized a preclinical PERCIST paradigm to complement clinical standards. Considerations in this effort included variability in tumor growth rate and tumor size, solid tumors versus tumor heterogeneity and a necrotic phenotype, and optimal selection of tumor slices versus whole tumor. <b>Methods:</b> A test-retest protocol was implemented to optimize the reproducibility of <sup>18</sup>F-FDG PET SUV thresholds, SUV<sub>peak</sub> metrics, and preclinical PERCIST parameters. In assessing response to therapy, <sup>18</sup>F-FDG PET imaging was performed at baseline and 4 d after therapy. The reproducibility, accuracy, variability, and performance of imaging metrics to assess response to therapy were determined. We defined an index called the Quantitative Response Assessment Score to integrate parameters of prediction and precision and thus aid in selecting the optimal image metric to assess response to therapy. <b>Results:</b> Our data suggest that a threshold of 25% of SUV<sub>max</sub> (SUV<sub>25</sub>) was highly reproducible (<9% variability). The concordance and reproducibility of preclinical PERCIST were maximized at α = 0.7 and β = 2.8 and exhibited a high correlation with SUV<sub>25</sub> measures of tumor uptake, which in turn correlated with the SUV of metabolic tumor. <b>Conclusion:</b> The Quantitative Response Assessment Score favors SUV<sub>25</sub> followed by SUV<sub>peak</sub> for a sphere with a volume of 14 mm<sup>3</sup> (SUV<sub>P14</sub>) as optimal metrics of response to therapy. ich freue mich sehr, Ihnen mit der vorliegenden Septemberausgabe der LRO ein abwechslungsreiches und interessantes neues Heft aus unserem schönen Fachgebiet vorstellen zu dürfen. Eingangs werden aus unserer Sicht relevante Publikationen diskutiert, so beispielsweise die Behandlung des gutartigen Lagerungsschwindel und deren Auswirkung auf Angststörungen [1]. Eine türkische Arbeitsgruppe konnte herausarbeiten, dass Angststörungen im Zusammenhang mit Schwindel mit dem Grad der Bildung der Patienten korrelieren. Des Weiteren wird eine Arbeit zur Otoplastik in Zusammenhang mit psychosozialen Problemen diskutiert, wobei der Operation ein äußerst positiver Effekt auf diesen Parameter zugestanden wird [2]. Aus Sicht von Kollegen Lindemann aus Ulm wird dieser Zusammenhang deutlich unterstützt. Schließlich wird das Thema Patientenzufriedenheit nach Lokalanästhesie bei ambulanten Kehlkopfoperationen berührt [3]. Bei dieser Studie war die transtracheale Lidocain-Installation die von Patienten und Operateur bevorzugte Applikationsform. Herr Kollege Grundmann aus Hamburg unterstützt diese Feststellung. Kollege Nawka aus Berlin beschäftigt sich mit Glottisdefekten nach Chordektomie in der Rubrik „Sehen und Verstehen [4]“. In einer interessanten Übersichtsarbeit von Kollegen Koch aus Uerdingen wird das Thema Arztbewertungsportale – Tipps und Tricks dargestellt [5]. Das Thema beschäftig zunehmend auch Anwälte und Gerichte, da einerseits viele Kollegen*innen sich gegenüber unberechtigten anonymen Negativbewertungen aktiv schützen und andererseits auch Patienten über die Computer-ID rückverfolgbar sind. Es lohnt sich also, sich ein wenig Kompetenz zu diesem Thema anzueignen. Mit dem Einfluss der Alltags relevanz von APHABu-Fragen auf deren Antwort befasst sich Frau Storz und Kollegen aus Lübeck [6]. Frau Kuhr und Kollegen aus Marburg legen eine Originalie mit dem Titel „Analyse der Notfallpatienten in einer Universitäts-HNONotfallambulanz“ vor. In der Rubrik „Der interessante Fall“ wird über den seltenen Fall einer beid seitigen periorbitalen nekrotisierenden Fasziitis (PONF) berichtet [8]. In der Rubrik Gutachten und Recht beschäftigt sich Kollege Brusis mit der Begutachtung einer vermuteten Rundfensterruptur [9]. Frau Kollegin Just aus unserer Leipziger Klinik stellt die chirurgischen Therapieoptionen bei obstruktiven schlafbezogenen Atemstörungen und Schnarchen in der Rubrik Facharztwissen HNO vor [10]. Mit den Fragen für die Facharztprüfung [11] sowie einem Auszug aus der Operationslehre von Rettinger und Guntinas-Lichius zu plastisch rekonstruktiven Gesichtschirurgie findet das Heft einen runden Abschluss. Das Thema „Extraktionsmethoden“ ist sogar zum allerersten Mal bei uns vertreten und auch wenn der eine oder andere erfahrene Kollege sich wundert, was er denn nach langen Jahren Praxistätigkeit bei einem Basisthema noch lernen soll, wird er sich doch über die neuen Aspekte freuen, unter denen heute die Entfernung von Zähnen stattfinden soll. Gerade weil als Folge der Präventionsbemühungen der letzten Jahrzehnte auch immer weniger extrahiert wird, ist eine Auffrischung der (Er-)Kenntnisse durchaus angesagt. Herr Kollege Strietzel von der Charité Berlin hat uns einen bunten Blumenstrauß aus seinem chirurgischen Portfolio zum Thema Extraktion zusammengestellt. Unter dem Gesichtspunkt des weitestmöglichen Erhaltes der anatomischen Strukturen ist die schonende Zahnentfernung unabdingbar, um die bestmöglichen Bedingungen für eine spätere implantologische Versorgung zu erreichen. Hier gilt es nicht, bei der Zahnentfernung Geschwindigkeitsrekorde zu brechen, sondern den anatomisch-physikalischen Gegebenheiten Zeit zu geben und den Zahn so atraumatisch wie möglich zu extrahieren.
