This study examined the prevalence of health problems and their associated factors among Indonesian farmers.
Methods
A cross-sectional study was conducted among 179 farmers who completed a self-administered questionnaire and physical examination. The data were analyzed using descriptive, comparative, and multinomial logistic regression analyses.
Results
The prevalence of varying health problems was 28.5% underweight, 10.6% overweight, 62.6% anemia, and 50.3% joint and bone pain. These results showed that being older and drinking coffee increased the likelihood of being underweight, while having less than 30 min of recess per working period and working for more than 5 days per week decreased the likelihood of being overweight. Meanwhile, being a younger male and working for less than 5 days per week decreased the risk of anemia. Furthermore, older age and less than 30 min of recess per working period were associated with increased joint and bone pain.
Conclusions
We recommend the provision of screening programs and health education programs, including dietary programs and physical and exercise programs, by the occupational health nursing (OHN) program, to prevent and reduce health problems in the agricultural sector.

Objective
To compare the feasibility and applicability of predicting the prognosis of patients using the Early Warning Score (MEWS) system and the Acute Physiology and Chronic Health Evaluation (APACHE II) system in the Emergency Department.
Methods
Using a prospective study method, the APACHE II and MEWS data for 640 patients hospitalized in the Emergency Internal Medicine Department were collected. The prognoses, two scores to predict the corresponding prediction index of sensitivity, specificity and positive predictive value for the prognosis, the negative predictive value and the ROC curve for predicting the prognosis were analyzed for all patients.
Results
In the prediction of the risk of mortality, the MEWS system had a high resolution. The MEWS area under the ROC curve was 0.93. The area under the ROC curve for the APACHE score was 0.79, and the difference was statistically significant (Z = 4.348, P < 0.01).
Conclusions
Both the MEWS and APACHE II systems can be used to determine the severity of emergency patients and have a certain predictive value for the patient's mortality risk. However, the MEWS system is simple and quick to operate, making it a useful supplement for APACHE II score.

Background
Improving and sustaining the quality of nursing care is an intractable and persistent challenge. The patient experience of nursing care can give a different perspective on nursing quality and help clinical nursing staff to direct quality improvement. Attempts to identify what is patient experience, the relationship with nursing care, and the application of patient experience in nursing quality improvement practice, in order to provide advices for constructing the quality standards and quality improvement strategies of nursing care.
Methods
We conducted a literature review of original research publications. The database platforms Pubmed, Web of Science, China National Knowledge Infrastructure (CNKI) and Wanfang were searched from inception until end of August 2015. After screening retrieved articles, 40 sources (articles and organizational websites) were selected for analysis for the connotation, development and application of patient experience in nursing quality researches.
Results
Our study identified several concepts about patient experience, roughly sorted out the development path of patient experience in worldwide and the application of patient experience in nursing quality improvement. It points out four precautions when applying patient experience in nursing quality improvement, which are differentiating patient satisfaction and patient experience, choosing appropriate data collection method and appropriate feedback time, and the last but the most important, applying theories to actual clinical practice.
Conclusions
Professional indexes are important to maintain the care quality, but it cannot fully reflect quality of nursing care, which needs patient experience as the supplement. Nursing staffs need to make more efforts to enhance patient's nursing care experience, and apply the research results to clinical practice, and finally make patient-centered care come true. For future study, the evaluation system and management strategies about patient experience need to be developed to guide nursing quality improvement.

Aim
the aim of this methodological article is to reflect on and extend current understandings of the possibilities of narrative inquiry research giving voice to students, and to expand the power of story by sharing the philosophical, theoretical, and methodological considerations of narrative inquiry in an international education context.
Background
there has been much discussion about the need in providing a ‘voice’ to people across the society, who feel marginalised in many contexts, including international students. There is limited research about Chinese students studying in Australia. In particular, the learning experience of Chinese nursing students has not been fully explored nor understood.
Discussion
to enhance teaching and learning in international education contexts, and to cater better to international students, it is important to understand their experiences and perspectives. There is no better way to achieve this level of understanding than to let students' voices be heard, to let them speak for and about themselves because reality exists within these students' perceptions.
Conclusions
in the context of international education, narrative inquiry as a research methodology, when used with sensitivity and reflexivity, through the power of stories, offers a new dimension in the international education research.

Objective
This study aimed to explore adherence to and beliefs about immunosuppressive medication using the Necessity-Concerns Framework (NCF) in liver transplant recipients.
Methods
A cross-sectional study performed in recipients who were at least 3 months post liver transplantation. A convenience sample of 243 recipients was recruited. Self-reported medication adherence was measured by the Basel Assessment of Adherence with Immunosuppressive Medication Scale (BAASIS). The NCF was operationalized using the Beliefs about Medication Questionnaire (BMQ) to assess the beliefs about necessity and concerns with taking immunosuppressive medication.
Results
One-hundred-forty-five liver transplant recipients were non-adherent (59.67%). Compared to adherers, non-adherers had lower beliefs regarding the necessity of taking immunosuppressive medication and lower scores on the necessity-concerns different from adherers.
Conclusions
Non-adherence is common in liver transplant recipients. Non-adherers hold beliefs that are different from adherers. Efforts to increase adherence should be made by targeting medication beliefs.

Objective
By constructing a training system of quality evaluation standards for emergency nurse specialist (ENS), we can ensure smooth operations and quality training for ENS.
Methods
First, the frame structure of indicators was designed on the basis of system theory and the balanced scorecard method. Meanwhile, corresponding quantitative standard indicators were compiled through literature analysis and a review of training characteristics. Next, screening indicators were collected through consultation with experts and statistical calculations. The indicators weight coefficient was calculated using the analytic hierarchy process (AHP). Finally, indicators were validated in two groups of nurses in two different training courses.
Results
(1) We created a three-level indicator system: level-Ⅰ dimensions have 4 indicators, while level-Ⅱ dimensions and level-Ⅲ dimensions have 13 and 34 indicators, respectively; (2) The coefficient of expert's judgment is 0.840, familiarity is 0.914 and authority is 0.877, and the three rounds of coordination coefficient are 0.456, 0.553 and 0.715, respectively; (3) There are at least 56 indicators in alternative quantitative standards; and (4) The alpha reliability value of the indicator system in the two training course had no significant difference (P > 0.05). The same result was observed when examining two groups of nurses in one training course (P > 0.05).
Conclusions
This study established a training system of quality evaluation standards for emergency nurse specialists that is objective, reliable, easy to operate and representative according to scientific selection and verification. This system can therefore provide a basis for quality evaluation and targeted improvement for ENS training in addition to promoting health.

Objectives
To systematically evaluate the curative effects of traditional Chinese medicine retention enemas for patients with radiation proctitis.
Methods
Reports on randomized controlled trials (RCTs) on traditional Chinese medicine retention enemas in the treatment of radiation proctitis were searched in the Cochrane Library, PubMed, EMBASE, EBSCO, ISI, CNKI, VIP, WanFang Data and CBM databases. The publication dates were from inception to May 2014, and a literature screening, data extraction, quality evaluation and cross checks were independently conducted by two reviewers in accordance with the inclusion and exclusion criteria. A meta-analysis was then carried out using RevMan 5.3 software.
Results
In total, 14 RCTs were included, and of these, only 10 RCTs involving 702 patients were included in the meta-analysis. The meta-analysis showed that the total efficacy of traditional Chinese medicine retention enemas was higher for patients with radiation proctitis compared with that in control group, RR = 4.83, 95% CI (2.98, 7.84), P < 0.000 01; the results from four studies indicated that the improvement of clinical symptoms due to traditional Chinese medicine retention enemas was higher than that of the control group.
Conclusions
The implementation of traditional Chinese medicine retention enemas could improve the total efficacy of treatment and provide relief in patients with radiation proctitis.

Background
Activity in a favorable environment excites the brain on a bodily and cognitive level and is beneficial for brain plasticity in dementia patients. Although dementia is an incurable disease, its progression can be slowed with exercise and some patient functionality may be improved through physical exercise. The objective of this meta-analysis was to assess the effect of physical exercise on patients with dementia.
Methods
The Pubmed, EMbase, Web of Science, Cochrane Library, China National Knowledge Infrastructure (CNKI), VIP and Wan Fang databases were searched for randomized controlled trials (RCTs) through July 2016. References of eligible citations were also searched by hand. Articles were screened and data were extracted. The methodological quality was independently assessed by two reviewers. Then, a meta-analysis was performed by RevMan 5.3.
Results
A total of 14 RCTs, involving 1546 participants, were included. The pooled results suggested that physical exercise effectively improved the Time up and go (seconds) [MD = −2.87, 95%CI (−3.25, −2.5)], Function Reach (cm) [MD = 4.25, 95%CI (3.52, 4.98)], Cadence, steps/min [MD = 12.71, 95%CI (6.92, 18.51)], NPI-caregiver total score [MD = −2.33, 95%CI (−3.65, −1.01)] and Berg Balance Scale [MD = 3.62, 95%CI (1.51, 5.73)].
Conclusions
Physical activity training effectively improved the condition of dementia patients. Physical activity training effectively improved the condition of Time up and go (seconds), NPI-caregiver total score, Function Reach (cm), Cadence, steps/min and Berg Balance Scale. Hence, physical activity training offers many advantages for dementia patients, including improving their balance ability, athletic ability and relieve caregivers' burden.
