Novel Coronavirus COVID-19 and Its Diagnosis and Treatments 新型冠状病毒COVID-19及其诊断和治疗

Recently, at the end of 2019 in Wuhan, China found a novel flu like Corona virus (COVID-related to the severe acute respiratory syndrome (SARS) in 2002 and Middle East respiratory syndrome (MERS) in 2012 and so many cases it confirmed that, it transmitted through human-to-human and spread all over world. There is no proper medicine and vaccine on this novel coronavirus COVID-19. The Economy of so many countries going on declined state due to this novel coronavirus COVID-19. It is challenge to scientists to find out actual medicine and vaccine on this novel coronavirus COVID-19. The present review is an attempt made in the respect of highlighting the some important information regarding detection and treatments on Novel coronavirus COVID-19.

Celiac Disease and Its Clinical Manifestations 乳糜泻及其临床表现

Celiac disease (CD) is associated with activation of the immune response toward specific protein commonly known as gluten, Which causes damage to the small finger-like projections called villi. Samuel was the first person who describes the disease in the year 1887. The given review article describes various factors which are associated with CD such as genetic factors, environmental factors and proteins. Chronic diarrhea, Hypochromic anemia, Short stature, Bone loss, Recurrent abortions, Hashimoto’s disease, etc are some common clinical manifestation. Along with clinical manifestations, we will also discuss the diet for the patients and treatment approaches.

COVID-19, caused by severe acute respiratory syndrome coronavirus (SARS-CoV-2), is now a worldwide spreading pandemic. Most of the attention is directed to the upper and lower respiratory airways, however, it appears that the gastrointestinal tract is also inoculated and infected. In this regards, celiac disease patients are a potential high-risk group for the corona virus infection and the endoscopist communities should be aware of their professional risks.

Enhanced Recovery After Surgery (ERAS) Protocols is Extremely Beneficial in Liver Surgeries - A Meta-Analysis 增强术后恢复(ERAS)方案在肝脏手术中非常有益-一项荟萃分析

BACKGROUND: Enhanced recovery after surgery (ERAS) programs aim to improve postoperative outcomes. This meta-analysis aims to evaluate the impact of ERAS programmes on outcomes following liver surgeries. METHODS: EMBASE, MEDLINE, PubMed and the Cochrane Database were searched for studies comparing outcomes in patients undergoing liver surgery utilizing ERAS principles with those patients receiving conventional care. The primary outcome was the occurrence of 30-day morbidity and mortality. Secondary outcomes included length of stay, functional recovery, readmission rates, time to pass flatus, blood loss and hospital costs. RESULTS: Ten articles were included in the meta-analysis. 30 days morbidity and mortality were significantly less in the ERAS group. Hospital stay, time to pass flatus, time to complete recovery and hospital costs were also significantly reduced due to ERAS protocols. Blood loss and readmission rates were also significantly less in the ERAS group. CONCLUSIONS: The adoption of ERAS protocols significantly reduced morbidity, mortality hospital stay, readmission rates, time to recovery, hospital costs, time to pass flatus, blood loss and readmission rates.

Celiac Disease Presenting as Hypokalemic Quadriparesis 乳糜泻表现为低钾性四肢瘫

Celiac disease is an autoimmune disorder induced by dietary gluten, characterized by a chronic inflammatory state of the small intestinal mucosa, resulting in villous atrophy that resolves with gluten free diet. A small number have classical symptoms and the majority present as atypical celiac disease with absent or few GI symptoms. Celiac crisis is a rare life threatening complication of celiac disease which needs prompt recognition and intervention. We report a rare case of Celiac disease which presented as Celiac crisis with hypokalemic quadriparesis.

Sprue-Like Intestinal Disease Following Crohn’s Disease 克罗恩病后的豆芽样肠道疾病

A 48-yr old female was reviewed for ongoing symptoms of diarrhea and weight loss following a total colectomy for colitis. Pathological review of the surgical specimens revealed changes of Crohn’s colitis. Subsequent investigations after the colectomy revealed endoscopic evidence of duodenal mucosal scalloping, negative celiac serological studies and moderate to severe changes in villous architecture including crypt hyperplasia with complete villus atrophy and increased numbers of intra-epithelial lymphocytes. No changes were present in the distal small intestine. Over the course of almost a decade, measures directed towards symptom relief were effective. However, in spite of a strict gluten-free diet, repeated endoscopic biopsies of the proximal small bowel remained severely abnormal with persistent crypt hyperplastic villus atrophy. “Sprue-like” intestinal disease occurs in Crohn’s disease limited to the duodenum, mimicking biopsy changes of celiac disease and may represent a post-colectomy complication in Crohn’s disease.

Pathological review of the surgical specimens revealed changes of Crohn’s colitis. Subsequent investigations after the colectomy revealed endoscopic evidence of duodenal mucosal scalloping, negative celiac serological studies and moderate to severe changes in villous architecture including crypt hyperplasia with complete villus atrophy and increased numbers of intra-epithelial lymphocytes. No changes were present in the distal small intestine. Over the course of almost a decade, measures directed towards symptom relief were effective. A Rare Case of Celiac Presenting at an Atypical Age with Multisystem Involvement 罕见的乳糜泻出现在非典型年龄并累及多系统

Celiac Disease is an autoimmune disorder due to hypersensitivity to gluten. Gluten free diet usually reverses the pathological changes but in 7-30 % of cases when there is no reversal one should look for the evidence of Microscopic colitis. Celiac disease is also associated with multiple haematological and renal abnormalities and here we report an atypical case of Celiac presenting in an unusual age with multisystem involvement.

Prevalence of Celiac Disease Autoimmunity in Ethiopian Pregnant Women: A Cross Sectional Study from the Oromia region 埃塞俄比亚孕妇乳糜泻自身免疫的患病率:来自奥罗米亚地区的横断面研究

Celiac disease is a chronic small bowel disease induced by ingestion of gluten in genetically susceptible individuals affecting 1% among Caucasians in the Western world. The prevalence of celiac disease is still unknown in most developing countries, especially in Africa which suffer from lack of resources to perform screening of the general population. The aim of this study was to determine the prevalence of celiac disease autoimmunity in women undergoing antenatal care in selected Ethiopian health institutes. A total of 1942 pregnant women were included at median 25 (range 15-45) years of age who were attending antenatal care at 14 health centers of Central and South-East Oromia regional state of Ethiopia. Serum samples were analyzed for both IgA and IgG autoantibodies against tissue transglutaminase (tTG) using radioligand binding assays. Celiac disease autoimmunity defined as testing positive for both of IgA-tTG and IgG-tTG. In all, 4 of 1942 (0.2%) were positive for IgG-tTG of whom one participant (0.05%) was positive for both IgA-tTG and IgG-tTG and defined as having celiac disease autoimmunity. Based on these results, it was concluded that celiac disease autoimmunity is expected to be less common among the female adult Ethiopian population compared with the expected prevalence in Caucasians.
