Introduction Tension-type headaches (TTH) are a significant health concern that demands precise diagnostic approaches and effective management strategies. A nuanced understanding of their pathophysiology and diagnostic challenges is not just pivotal, but a responsibility we share, for enhancing patient care. Aim  This article aims to explore the complex realm of TTH diagnosis and management, underlining the need for precision in diagnostics and the intricate balance of cost-effectiveness. It also delves into the diverse therapeutic options available for treating TTH, presenting a stimulating challenge for healthcare professionals, researchers, and medical students. State of Knowledge Initial diagnosis involves excluding secondary headaches by identifying red flags indicative of serious underlying conditions. A detailed medical history, complemented by patient-recorded pain diaries, enhances diagnostic accuracy. Ancillary procedures like neuroimaging, despite cost-effectiveness debates, are often preferred by clinicians to mitigate diagnostic risks. Validated tools such as the MIDAS and HIT-6 questionnaires provide valuable insights into headache-related disability. Differential diagnosis among headache types relies on recognizing distinctive clinical features. Summary (Conclusions) Effectively managing TTH necessitates a multifaceted approach, incorporating pharmacological, non-pharmacological, and lifestyle interventions tailored to individual patient needs. Collaborative efforts spanning legal, technological, and research domains are crucial for advancing headache diagnostics and therapeutics. While technological advancements offer promise, interdisciplinary collaboration remains fundamental in optimizing patient care outcomes.

Eating disorders in pediatric population, analysis of some of the most common risk factors 儿科人群饮食失调,分析一些最常见的风险因素

Introduction: This article reviews a number of studies that have sought to determine what are risk factors that may cause eating disorders in the pediatric population, taking into account the age and gender of the subjects. This study focuses on issues related to purging, bulimic behaviors, and psychological orthorexia and what impact the most common risk factors for these behaviors have in the pediatric population. Aim of study:The aim of the study is to present a comprehensive review of some of the most common risk factors predisposing to the development of eating disorders in the pediatric population, and how these factors have a direct impact on each other and are intertwined, by using available knowledge, paying particular attention to a new potential risk factor - social media. Materials and methods: Materials used in this study were found in the PubMed database, using the following keywords: ,,Social media”, ,,Mass media”, ,,Peers”, ,,Family”, ,,Orthorexia nervosa”, ,,Social media”, ,,Children”, ,,Adolescence”. Conclusion: Eating disorders are a growing problem. Our research focuses on potential risk factors, with the greatest emphasis on a relatively new risk factor: social media and screen time, and how these two factors influence young minds, what impact they have on the eating habits of children and adolescents, and what can be done to prevent eating disorders in the pediatric population.

Assessment of balance and risk of falls in people over 60 years old 评估 60 岁以上老年人的平衡能力和跌倒风险

Introduction: Falls of the elderly are a significant problem for public health and healthcare. They are also included in the geriatric giants, and the assessment of balance and risk of falls is an important part of the comprehensive geriatric assessment (CGA). It is estimated that almost one in three people over the age of 65 living independently in a community suffers one fall per year. These estimates increase with age, and in people older than 80 years, every second senior may experience at least one fall. The aim of the study was to assess the balance and risk of falls in people over 60 years of age living independently in society. Materials and methods: The study enrolled 40 seniors over the age of 60 who agreed to participate in the study and met the inclusion criteria. Three functional tests were used to assess balance and the risk of falls among seniors: the Timed Up and Go (TUG), the Four Square Step Test (FSST) and One-leg standing (OLS). Result: The study shows that as age increases, subjects perform worse on the Timed Up and Go rho = 0.38 test; p < 0.05 and in the Four Square Step Test rho = 0.53; p < 0.01. No statistically significant differences were observed between genders or age groups in the One Leg Standing test (OLS), although older people scored worse than younger age groups. There were no statistically significant associations between performance test results and BMI. The average results of the clinical tests show that the seniors in the study had good balance and a low risk of falling.

Introduction and objective: Diabetes represents one of the most significant healthcare challenges of the 21st century. The disease is frequently asymptomatic, with patients often remaining undiagnosed for extended periods and failing to seek assistance from healthcare professionals. A sedentary lifestyle, excessive caloric intake, and a lack of physical activity contribute to the development of obesity, which in turn leads to carbohydrate metabolism disorders. In this study, we will examine the relationship between diabetes and obesity, and discuss the drug semaglutide, which is used to treat type 2 diabetes and also promotes weight loss. Review methods: Review and summary of research studies available in open-source format on Google Scholar, PubMed. Abbreviated description of the state of knowledge: Numerous clinical studies have demonstrated the efficacy of semaglutide in improving lipid profiles and promoting weight loss. This effect is believed to be mediated by the drug's ability to slow gastric emptying. It is evident that semaglutide, a glucagon-like peptide-1 (GLP-1) analogue, exerts a beneficial effect on carbohydrate parameters and helps to delay the occurrence of complications associated with diabetes. Summary: A review of clinical reports and conducted studies indicates that semaglutide has the potential to enhance parameters that directly or indirectly impact the quality of life of patients with diabetes. However, the utilisation of the aforementioned pharmaceutical is associated with adverse effects, which may impede patient-physician collaboration and potentially precipitate the development of new health complications in patients. One of the effects of semaglutide treatment is a reduction in appetite, which contributes to weight loss. This property of the drug makes it suitable for use in patients with excessive body weight.

Assessment of physical fitness among people over 60 years of age 对 60 岁以上老年人的体能进行评估

Introduction: An ageing of population is becoming more and more common. With age, the functionality and efficiency of the body decreases. As a result, the time spent by seniors on physical activity is shortened and physical fitness decreases. For this reason, the physical fitness of seniors should be regularly checked and they should be encouraged to take up physical activity. There are many proven and reliable tests used to assess physical fitness.             Materials and methods: The study group consisted of 40 people (31 women and 9 men), aged 60-78 years. The tests used in following study were: the SPPB questionnaire, handgrip strength test, and arm and calf circumference measurements. An attempt was made to find sociodemographic factors that could influence the level of physical fitness. It was checked whether physical fitness and physical activity depend on age, gender and BMI. It was checked how much time older people spend on physical activity during the day and week. The study confirmed that physical fitness decreases with age.             Results: Tests and interviews have shown that people who spend more than 60 minutes a day on physical activity achieve higher scores on the SPPB questionnaire. These results were applicable to both women and men. Studies in a larger and more diverse population would need to be repeated in order to further confirm these results and draw more conclusions.

Psoriatic arthritis - Tofacitinib as a new treatment 银屑病关节炎--托法替尼是一种新疗法

Introduction:JAK inhibitors are used in the treatment of psoriatic arthritis when there is a lack of effective response or intolerance to first-line drugs or their use must be discontinued due to the presence of side effects. JAK inhibitors inhibit the JAK-STAT signaling pathway which plays a significant role in the pathogenesis of many inflammatory and autoimmune diseases. This mechanism leads to a reduction in the level of pro-inflammatory cytokines which causes rapid improvement in the patient's clinical condition. Tofacitinib is the best-known drug in this group; its use carries an increased risk of cardiovascular events and reactivation of the varicella-zoster virus. Regular monitoring of patients results in faster detection of the first signs of undesirable effects and the cessation of their progression. The drug's safety profile is acceptable and the benefits outweigh possible complications. Aim of  the study: The aim of the study is to summarize the available knowledge about tofacitinib treatment in psoriatic arthritis. The way of work, effectiveness of treatment and potential side effects were summarized and described. Materials and methods: The literature available in PubMed database was reviewed using the following keywords:“Psoriatic arthritis”, “Tofacitinib”, “JAK inhibitors”, “JAK-STAT” Conclusion: Tofacitinib treatment in rheumatology is used in psoriatic arthritis. The rapid improvement in the clinical condition of patients treated with JAK inhibitors is due to their direct impact on the modulation of the pathogenesis of the disease. The predictable benefits of therapy outweigh the side effects which can be detected at an early stage with regular monitoring of the patient.

In Lyme disease, AI models have been developed to identify patterns in clinical data, enhancing early detection and accuracy. Studies have focused on using AI to interpret complex serological results and clinical symptoms more effectively than traditional methods. Additionally, AI has been utilized to analyze geographical and epidemiological data to predict Lyme disease risk areas, aiding in preventive strategies.Summary: AI holds significant promise in transforming Lyme disease diagnostics by increasing the speed and accuracy of detection. These technologies not only help in overcoming the limitations of current serological testing but also provide a framework for predictive analytics in epidemiology. This review aims to consolidate current knowledge on the mechanisms of action and clinical applications of phototherapy, with a focus on enhancing understanding among clinicians and researchers.State of Knowledge: Phototherapy is particularly effective for chronic conditions such as psoriasis, vitiligo, and atopic dermatitis, where traditional therapies might not suffice. The mechanism involves modulation of immune response and inhibition of cellular proliferation, achieved through various wavelengths, including UVA and UVB. Recent advancements have introduced narrow-band UVB, which offers targeted therapy with fewer side effects compared to broad-band UVB. Moreover, the review explores innovations such as LED-based therapies and photodynamic therapy, expanding the spectrum of phototherapeutic options.Summary: Phototherapy remains a vital and evolving treatment modality in dermatology, offering safe and effective management for multiple skin disorders. Its success hinges on precise application and adherence to treatment protocols. This review aims to synthesize current knowledge on the health risks and complications faced by marathon runners, providing crucial insights for both amateur and professional athletes.State of Knowledge: Marathon running imposes significant stress on the cardiovascular system, musculoskeletal structure, and metabolic processes. Issues such as acute myocardial injury, overhydration leading to hyponatremia, and musculoskeletal injuries are prevalent. The review also examines less common but severe risks like rhabdomyolysis and marathon-related cardiac arrests. Despite the robust fitness levels of runners, these health risks remain a serious concern, necessitating preventive strategies and proper health monitoring.Summary: Marathon runners must be aware of the potential health risks associated with prolonged endurance activities. Precautionary measures, including tailored training programs, dietary adjustments, and regular medical check-ups, are essential to mitigate these risks. Background: Creatine, a naturally occurring compound plays a crucial role in cellular energy metabolism, particularly in tissues with high energy demands like muscles and the brain. Beyond its well-established role in physical performance enhancement, recent research has revealed its potential benefits for cognitive functions and mood regulation. This review paper aims to comprehensively analyze the existing literature on creatine supplementation's effects on cognitive functions and mood. State of Knowledge: Findings suggest that creatine supplementation enhances memory, attention, and executive functions, especially under conditions of stress or sleep deprivation. Moreover, it exhibits mood-stabilizing effects, potentially offering relief for symptoms of depression and fatigue. Creatine's ability to augment brain energy metabolism, alongside its neuroprotective properties, underlies its cognitive and mood-enhancing effects. While generally safe, caution is advised, particularly in populations with kidney diseases or those using medications affecting kidney function. Summary: Creatine emerges as a promising adjunct therapy for enhancing cognitive functions and promoting emotional well-being beyond its traditional role in physical performance.The review underscores the need for further research to elucidate the precise mechanisms of creatine's effects and explore its synergistic potential with other cognitive-enhancing supplements.
