Introduction: Lens luxation generally happens secondary to trauma and hereditary disorders into the vitreous body. However, spontaneous dislocation especially into the anterior chamber occurs rarely. We report an unusual case with spontaneous anterior dislocation of a posterior chamber intraocular lens (IOL) 4 years after cataract surgery. Case Presentation: A 42-year-old woman was referred to ophthalmology department with complaint of sudden onset blurring of vision in the left eye 4 years after cataract surgery. No history of trauma and systemic disorder was reported and no capsular fibrosis was found during eye examination. The patient underwent corneoscleral incision under general anesthesia and the IOL was extracted using a viscoelastic material. Conclusions: To our knowledge, this is the first reported case in the literature. This report may imply that IOL related factors could be considered for spontaneous dislocation of intraocular lenses.

Background: The fact that emotional dysregulation and early maladaptive schemas are considered as the defining core of conduct disorder has been validated through numerous studies. A strong association between the diagnosis of conduct disorder and callousunemotional traits has also been found by numerous studies. Objectives: This study was carried out in order to inspect the relationship between conduct disorder and emotion regulation, callousunemotional traits, and schema in male high-school students in Dezfoul, SW Iran. Methods: In the present descriptive correlational study, 320 male high school students were chosen in Dezfoul, using the multistage random sampling. Data collection was carried out using some instruments including the child behavior checklist (CBCL), difficulties in emotion-regulation scale (DERS) inventory of callousunemotional traits (ICUT) young schema questionnaire-short form. The collected data were analyzed using multivariate regression analysis and the pearson correlation coefficient. Results: The results indicated a meaningfully positive correlation between emotion regulation, callousunemotional traits and schema with conduct disorder. In addition, the results of multivariate regression analysis showed that among the above-mentioned variables studied, callousunemotional trait was found to be the most effective predicting variable in conduct disorder (P < 0.001). Conclusions: The findings of this study are in full agreement with those of previous ones, and basically show a significant association between callousunemotional traits, emotion regulation and schema with conduct disorder.

Background: Many studies indicate that one of the leading preventable causes of premature death, disease and disability around the world is Tobacco use. Unfortunately, adolescents and young adults of colleges are often targeted of marketing by the tobacco industry. The aim of this study was to assess the relative frequency of tobacco use and associated factors in medical students. Methods: This cross-sectional study has done among medical students in 2014 2015. Totally, 284 students from 4 levels (basic, extern, intern and residents) were selected by random sampling from each category. We used a checklist for collecting demographic information that was distributed among participants by a trained interviewer. SPSS-11.5 software was used for data analysis andsignificance level was considered < 0.05. Results: Among 284 medical students, 17 (6%) smoked cigarette and 3 (1.05%) used illicit substance. All of the users were male, 15 (88%) of which were single. 1 (1.4 %) of the basic level students, 6 (6.9 %) of the externs, 9 (14.8 %) of the Interns and 1(1.6 %) of the residents used cigarette but the differences among these groups were not significant (P = 0.36). Generally, 13 (76.5%) of the users resided in dormitory and the others lived in parental home, which shows a significant difference among these groups (P = 0.01).The logistic regression indicated stage level was positively associated with cigarette use (P < 0.007). Conclusions: In this study, we evaluated how several environmental factors may influence illicit substance and tobacco use. We found an association between living in a dormitory and smoking cigarette, so it is an important factor to be considered in program planning for new students who entered into this environment.

Osteoid osteoma near the joints is a rare case which is difficult to diagnose. Osteoid osteomas refer to solitary, benign, agonizing lesions of the bone with a nidus of 1.5 to 2 cm, which consists of osteoid, osteoblasts, and varying quantity of fibrovascular stroma. This study reports the case of 37-year-old man with a painful flexion contracture of left elbow for 18 months, then was diagnosed a peri-articular osteoid osteoma. Any attempt for movement, whether passive or active, exacerbated the pain. His conditions was treated as tennis elbow and treated for several months, but his pain persisted continuously. A subcortical osteoid osteoma was observed in fine-cut CT scan within the posterior cortex of the capitulum. Thus, a wide en bloc surgical excision of the nidus was implemented, with total mitigation of pain and quick return to daily activities. It is stressed that history of disease and detailed physical examination can help diagnosis.

Background: Many small trials showed a significant improvement in blood pressure following renal artery stenting in patients with severe atherosclerotic renal artery stenosis, but data on renal function improvement is more conflicting. Recently, few trials have been conducted to evaluate the effect of this procedure on Left Ventricular Mass (LVM) and Left Ventricular Mass Index (LVMI). Objectives: The aim of this study is to determine the effect of renal artery stenting on Blood Pressure, estimated Glomerular Filtration Rate (eGFR), Left Ventricular Mass (LVM), and Left Ventricular Mass Index (LVMI) in patients with severe atherosclerotic renal artery stenosis. Methods: This is a prospective interventional study performed on forty patients with ischemic heart disease and medication resistant hypertension, who had severe (≥ 70%) atherosclerotic renal artery stenosis and underwent renal artery stenting. Blood pressure, LVM, LVMI and eFGR before renal artery stenting and after six months were assessed in these patients. Results: There were significant reduction in systolic blood pressure (from 175.50± 17.28 mmHg to 137.30± 13.21 mmHg) (P < 0.001), and diastolic blood pressure (from 103.45 ± 8.91 mmHg to 84.30 ± 7.33 mmHg) (P < 0.001). Also, there were significant decrease in LVM (from 307.73 ± 108.13 g to 259.34 ± 92.17 g) (P = 0.004) and LVMI (from 174.70 ± 58.26 to 148.01 ± 49.77) (P = 0.004). LVM reduction was independent of SBP and DBP reduction (P = 0.376 and P = 0.196, respectively). Conclusions: Renal artery stenting reduces Blood pressure and leads to regression of LVM independent of blood pressure reduction. Regardless of baseline eGFR, our study failed to find a significant increase in glomerular filtration rate.

Background: Age of menarche is a sign of the beginning of the ability to reproduce and is usually a turning point for the development and maturity and puberty. Regulatory factors in the onset of menarche age are not well understood, but in developed countries, it is thought that about half of the variations in the onset age of menarche is associated with genetic factors. Objectives: To determine the association between menarche age, birth weight and the mother and older sister’s age of menarche; the subjects of this study were Shirazifemaleprimary, middle and high schools students in 2014 2015. Methods: This is a cross-sectional study, which was done on 2000 girls aged 9 18 years. In the first stage, sampling of schools in 4 regions was randomly assigned to clusters. Then, the students in each school were selected purposively. The questionnaires filled in by the participants contained demographic items, menarche age, and birth weight. Data were analyzed through SPSS (version16), using Chi-square test and ANOVA. Results: The majority of the subjects (n: 1294, 64.7%) had normal menarche age (11 14 years old).There was no relationship between menarche age and birth weight (P: 0.67).However, it was significantly associated with the students’ mother and older sister’s age of menarche (P: 0.001). Conclusions: According to the findings, menarche age was within the normal range (11 14 years) in the majority of the subjects. There was a significant positive association between menarche age in both mothers and girls. In other words; girls reach menarche age earlier than their mothers. Thus, and mother and older sister’s menarche age can be a proper criterion to predictit in girls.

Introduction: To assess the benefits of prophylactic ascorbic acid to reduce development of contrast-induced nephropathy (CIN) in patients undergoing percutaneous coronary interventions (PCIs). Methods: PubMed was searched with the search strategy of (vitamin C OR ascorbic acid) AND (kidney OR renal) AND (PCI OR percutaneous coronary Intervention OR cardiac OR heart). There was no date and language restriction for the selection of the articles. All the randomized controlled trials (RCTs) which investigated the efficacy of AA on reducing the incidence of CIN were included. Totally 267 articles were found at the initial search; however, only 10 RCTs were eligible to be included. Odds ratio is presented for each of the articles as the effect size. Conclusions: Controversial findings were reported on the efficacy of AA on reducing the CIN development; due to various limitations of these articles, there is still great debate among the cardiology and radiology communities, which increases the need for further researches.

Amini","doi":"10.5812/RIJM.37341","DOIUrl":"https://doi.org/10.5812/RIJM.37341","url":null,"abstract":"Introduction: To assess the benefits of prophylactic ascorbic acid to reduce development of contrast-induced nephropathy (CIN) in patients undergoing percutaneous coronary interventions (PCIs). Methods: PubMed was searched with the search strategy of (vitamin C OR ascorbic acid) AND (kidney OR renal) AND (PCI OR percutaneous coronary Intervention OR cardiac OR heart). There was no date and language restriction for the selection of the articles. All the randomized controlled trials (RCTs) which investigated the efficacy of AA on reducing the incidence of CIN were included. Totally 267 articles were found at the initial search; however, only 10 RCTs were eligible to be included. Odds ratio is presented for each of the articles as the effect size. Introduction: Aortic surgery has made much progress recently. Yet the combination of aortic surgery and coronary artery bypass grafting has remained a major issue for the surgeons. Patient and Observation: Herein, we present a case of severe aortic insufficiency with aortic aneurysm concomitant with single coronary artery disease on the right coronary artery. The patient underwent Bentall procedure for the correction of severe AI and aortic aneurysm. Subsequently, due to the lack of proper site for proximal anastomosis of the saphenous vein graft on the ascending aorta, we decided to perform coronary-coronary artery bypass for the right coronary artery lesion instead of conventional implantation of saphenous vein graft on the ascending aorta. Conclusions: Coronary-coronary artery bypass could be performed safely for special conditions like porcelain aorta and when the ascending aorta is not suitable for the proximal saphenous vein graft.
