Objectives. To study the effect of toxoplasmas on the changes in the expression of protooncogenes BIRC5, ERBB-2/HER2-NEU, GLI, VEGF and the anti-oncogene TP53 in rats in an experiment depending on the infection dose and the period of parasite development. Material and methods.The experiment was conducted on female Wistar rats to determine changes in the expression of the protooncogenes survivin (BIRC5), epidermal growth factor (ErbB-2/HER2-Neu), GLI 1, vascular endothelial growth factor (VEGF) and the anti-oncogene TP53 in comparison with the reference genes β-actin (ACTB) and GAPDH by means of PCR analysis in the tissues of 10 healthy and 120 animals invaded at different doses. Statistical comparison of the results of all groups was drawn with the data of the «control» series (healthy animals, biopsies of the lungs, liver, spleen, brain). The results obtained in the experimental groups were as follows: the infection dose of 25 toxoplasma tachyzoites per 1 g of the animal body weight (5000 tachyzoites per female) and the infection dose of 50 toxoplasma tachyzoites per 1 g of the animal body weight (10000 tachyzoites per female), then they were also compared with each other. Statistical processing of the obtained data was carried out using the program Statistica 10.0. The differences were considered to be reliable at a significance level of less than 0.05 (p<0.05). Results. Toxoplasma was found to cause an infection dose-dependent increase in the expression of the protooncogenes survivin (BIRC5), epidermal growth factor (ErbB-2/HER2-Neu), GLI, vascular endothelial growth factor (VEGF) and a change in the strength expression of the anti-oncogene TP53 at all stages of the parasite development. Conclusions. Experimental toxoplasmosis alters the expression of the protooncogenes survivin (BIRC5), epidermal growth factor (ErbB-2/HER2-Neu), GLI, vascular endothelial growth factor (VEGF), and the anti-oncogene TP53 in the tissues of the intermediate host.

Piperidine is one of the most common heterocycles, and its derivatives are found in many pharmacological groups, including neurotropic drugs. These compounds are numerous among analgesics, and, in addition to “classical” promedol, fentanyl and its derivatives, the paper presents the results of studying new compounds with analgesic activity and piperidine cycle. Reviews of such piperidine antipsychotics as haloperidol and risperidone have been considered, and new compounds showing antipsychotic activity through their effects on dopamine and serotonin receptors have been elucidated. The data on the influence of methylphenidate on the brain in case of attention deficit/ hyperactivity disorder (ADHD) have been analyzed, which help to understand the disturbances occurring in this disease. Tiagabine has been considered as an antiepileptic agent reducing the number of seizures in resistant forms of partial epilepsy, as well as the activation of microglia and may be effective in neurodegenerative diseases. The last section is devoted to drugs for the treatment of Alzheimer’s disease (AD), namely donepezil, its modifications, and some new compounds potentially capable of inhibiting AD progression through the inhibition of Aβ42 protein synthesis.

The foundations of quantum physics have been laid by Max Planck, who suggested that energy couldn’t be absorbed and radiated continuously, but only in separate portions - these portions were called quanta. His ideas were confirmed in numerous physical experiments on the photo effect, the structure of the atom and atomic nucleus, brilliantly performed by Bohr and Rutherford. All this in the aggregate made it possible to eliminate the border between matter and waves, predicted by Louis de Broil. In this way the foundations of quantum mechanics were laid = Heisenberg and Schrödinger did this work. Many manifestations of quantum physics can already be observed in everyday life. These are optical quantum generators, computer CDs, and integrated circuits and lots and lots of this. In recent years, the researchers have drawn their attention to other quantum physics applications related to queries. By their design, this work will be carried out in the future by quantum computers. The article presents a short report on the quantum computer and the prospects for its use in quantum medicine.

Objectives. To assess the diagnostic value of urinary lipocalin associated with neutrophil gelatinase (uNGAL) as a monomarker, as well as in combination with other indicators, in acute kidney injury (AKI) diagnosing in the early postoperative period in cardiac surgery patients. Material and methods. The analysis of 73 case histories of ICU patients after cardiac surgery in the conditions of cardiopulmonary bypass was performed. AKIN criteria were applied to diagnose AKI. The primary end point of the study was the postoperative level of uNGAL, the secondary end point was the indexed value of uNGAL. Results. 31 (42.4%) patients developed AKI (according to AKIN score). uNGAL had a very good level of diagnostic significance in detecting AKI (AUC 0.849, p=0.001) and its optimal cutoff level was more than 48.2 ng/ml within the time frame up to 12 hours after surgery. The diagnostic efficiency of the uNGAL test was: specificity - 88.1%, sensitivity - 67.74%, positive predictive value - 80.8%, negative predictive value - 78.7%, positive likelihood ratio - 5.57, negative likelihood ratio - 0.37. The ratio of uNGAL to leukocytes in the terms up to 12 hours after surgery demonstrated an excellent level of diagnostic value (AUC 0.920, p=0.001). The diagnostic efficiency of this indicator (more than 5.5 μg/1*109) was: specificity - 94.74%, sensitivity - 73.68%, positive predictive value - 93.3%, negative predictive value - 78.3%, positive likelihood ratio - 14.0, negative likelihood ratio - 0.28. Conclusions. The diagnostic capabilities of uNGAL in detecting AKI in the early stages after cardiac surgery exceed the capabilities of serum creatinine traditionally used for this purpose. To improve the efficiency of AKI diagnosis in the early postoperative period, it is possible to use indexed indicators, for example, the ratio of uNGAL to leukocytes level in the patient’s blood.

Odjectives. To develop a scoring scale for assessing the risk of detecting right ventricle (RV) diastolic dysfunction (DD) of type II in chronic heart failure (CHF). Material and methods. In 2017-2018, a clinical and instrumental examination of 247 patients aged 40-86 years was performed on the basis of Health Care Institution «The 1st City Clinical Hospital» in Minsk, in 2019-2020, 129 patients at the age of 39-84 years were examined in the State Institution «Minsk Scientific and Practical Center for Surgery, Transplantology and Hematology». Study inclusion criteria: sinus rhythm, essential arterial hypertension (AH), chronic ischemic heart disease (IHD), CHF, informed voluntary consent of the patient to participate in the study. Exclusion criteria: primary mitral regurgitation, mitral stenosis, mitral valve repair or replacement, congenital heart defects, acute and chronic diseases of the lungs and kidneys. Results. The sum of points>52 obtained using a scoring scale that includes scientifically based ultrasound criteria for global cardiac remodelling: RV early diastolic filling index E/eʹ >4.08 (sensitivity (S) 81.1%, specificity (Sp) 56.9%) - 15 points, the amplitude of the systolic displacement of the lateral part of the tricuspid annulus in systole to the apex (TAPSE) ≤20 mm (S 74.1%, Sp 83.3%) - 49 points, pulmonary artery systolic pressure ˃32.6 mm Hg (S 85.2%, Sp 79.5%) - 31 points, indicates a high risk of detecting RV DD of type II in CHF (AUC 0.99, S 93.9%, Sp 100.0%). The reproducibility of the results is shown in an independent examination sample of patients with CHF with preserved left ventricular ejection fraction. (AUC 0.99, S 90.0%, Sp 100%, +PV 100.0%, - PV 34.5% with a cut-off threshold >52). Conclusions.When using the scale, the score is > 52, it is recommended to perform a comprehensive targeted assessment of global cardiac remodelling to determine RV DD of type II in a patient with CHF, complicating essential AH, chronic IHD.

Objectives. To analyze the possibility of using the technique of intraoperative ultrasound navigation on surgical treatment of patients with salivary stone disease of the parotid salivary gland (PSG). Material and methods. A retrospective analysis of three medical records of an in-patient treated at the department of maxillofacial surgery and operative dentistry in Gomel Regional Clinical Hospital during the period from 2019 to 2021 for salivary stone disease of PSG was made. Results. The results of clinical observation of treatment of a patient with sialolithiasis of the parenchymal part of the PSG are presented. Taking into consideration the complexity of surgical removal of sialolites of this localization, the patient was initially treated conservatively which was of no effect. Salivary stones were removed using intraoperative ultrasound navigation which made it possible to successfully perform the surgical intervention, reduce surgical trauma, create conditions for structural and functional recovery of the PSG and reduce the possibility of the disease relapse by improving intraoperative visual control. Conclusions. The use of the technique of intraoperative ultrasound navigation in the surgical treatment of patients with salivary stone disease of the parotid salivary gland allows to remove the concrement, minimizing the trauma to the tissues of the operated area and the organ itself, maintaining adequate blood flow and architectonics in it, which enables the reduction in the number of postoperative complications, relapses of the disease and positively affects the quality of patients’ life.

Salivary stones were removed using intraoperative ultrasound navigation which made it possible to successfully perform the surgical intervention, reduce surgical trauma, create conditions for structural and functional recovery of the PSG and reduce the possibility of the disease relapse by improving intraoperative visual control. Conclusions. The use of the technique of intraoperative ultrasound navigation in the surgical treatment of patients with salivary stone disease of the parotid salivary gland allows to remove the concrement, minimizing the trauma to the tissues of the operated area and the organ itself, maintaining adequate blood flow and architectonics in it, which enables the reduction in the number of postoperative complications, relapses of the disease and positively affects the quality of patients’ life.","PeriodicalId":23571,"journal":{"name":"Vestnik of Vitebsk State Medical University","volume":"4 1","pages":"88-93"},"PeriodicalIF":0.0,"publicationDate":"2021-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82010977","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objectives. To study the dependence of the formation intensity of microbial biofilms by isolates from patients on the severity and localization of the infectious process. Material and methods. The study conducted from 2016 to 2020 included 894 clinical isolates from 720 patients with infectious pathology of various localization and severety. The formation intensity of microbial biofilms by isolates received from these patients has been studied, depending on the severity and localization of the infectious process. Results. The property to form biofilms was found out in the overwhelming majority of tested isolates (86.6%). At the same time P. aeruginosa isolates had the maximum biofilm-forming ability, the weight of the microbial biofilm of which was 48.25; 30.1-70.2 mcg/well. The conducted studies have shown the dependence of biofilm formation on the severity of the disease in which isolates were isolated. A direct close relationship was found between the severity of the disease and the mass of microbial biofilm of P. aeruginosа and K. pneumoniae, a weak one - in Acinetobacter spp. (r=0.73, r=0.7 and r=0.35, p<0.05 accordingly). Conclusions. It can be assumed that intensive formation of biofilms by clinical isolates is an important factor of the aggressiveness and severity of the infectious process. Moreover, the conducted study has shown the dependence of biofilm formation on the severity of the disease in which isolates were isolated.

During the period of the 1950s and 1960s, higher medical education occupied one of the key positions in the health care system of the BSSR. After overcoming the medical and sanitary consequences of the Great Patriotic war and restoring fixed assets, the BSSR health care system faced further challenges to increase the amount of medical institutions and the number of doctors, and to improve the quality of providing medical care to the population. All these and many other tasks could not be solved without the comprehensive assistance of higher medical education. The purpose of the study is to show the key stages of the development of higher medical education in the BSSR in the field of practical health care in the 1950s and 1960s. The research was carried out on the basis of archival data stored in the collections of the National Archive of the Republic of Belarus, the State Archive of Vitebsk Region, scientific and historical literature from the National Library of the Republic of Belarus, the Vitebsk Regional Library named after V. I. Lenin, as well as information already introduced into scientific parlance. Special historical methods were used in this study: historical-genetic, historical-comparative, and historical-systemic.
