Application research on the sequence advance and retreat method in hospital performance appraisal system 序列进退法在医院绩效考核体系中的应用研究

In hospital performance evaluation, significant differences in the work objects, work content and economic operation of various departments result in the lack of indicators comparability between different departments. This has become a prominent problem that hospitals have been unable to effectively solve for a long time, and has also affected the fairness of performance distribution to a certain extent. Starting from the dilemma of cost-benefit evaluation, the authors put forward the basic concepts of the sequence advance and retreat method, and comprehensively elaborated the implementation steps and operating points of the performance assessment using the sequence advance and retreat method. It is pointed out that the sequence advance and retreat method has four characteristics: weakening the differences between departments by vertical and horizontal combination, avoiding the deviation of guidance by scientific comparison, highlighting the incentive effect by grade differentiation, and reflecting the advantages and disadvantages by end adjustment. The value of this method in the hospital performance appraisal system is discussed from three aspects: promoting the de-profitization process of hospitals, achieving fair distribution of hospital performance, and strengthening the guidance of hospital performance management. Based on such studies, this article provides new methods and ideas for building a more scientific and fair performance evaluation system. 在医院绩效评估中,各部门的工作对象、工作内容和经济运行存在显著差异,导致不同部门之间缺乏指标可比性。这已成为医院长期无法有效解决的突出问题,也在一定程度上影响了绩效分配的公平性。从成本效益评估的困境出发,提出了序列进退法的基本概念,并全面阐述了运用序列进退方法进行绩效评估的实施步骤和操作要点。指出,序列进退法具有四个特点:通过纵向和横向组合弱化部门之间的差异,通过科学比较避免指导的偏差,通过等级区分突出激励效果,通过末端调整体现优势和劣势。从促进医院去盈利进程、实现医院绩效公平分配、加强对医院绩效管理的指导三个方面论述了该方法在医院绩效考核体系中的价值。基于这些研究,本文为构建更加科学、公正的绩效评价体系提供了新的方法和思路。关键词:评价指标;绩效考核;绩效管理;医院管理

Status research of village doctors′ vulnerability in Shandong province based on the set pair analysis 基于集对分析的山东省乡村医生脆弱性现状研究

Objective To explore the current status of village doctors′ vulnerability in Shandong province in the context of ongoing healthcare reform. Methods A cross-section study was conducted from October 2015 to November 2015 based on a self-designed questionnaire for village doctors. The questionnaire included 6 parts: fundamental state, disturbance from surroundings, disturbance from job, emotional support, instrumental support and self-ability of village doctors. Mean and standard deviation were adopted to describe the level of disturbance and support. Quadrant analysis was adopted to analyze village doctors′ vulnerability. The set pair analysis was adopted to calculate the vulnerability index and sample cluster analysis was adopted to classify village doctors based on the vulnerability index. Results The total disturbance score was 3.39±0.46, and contribution from professional risk was the biggest(19.95%). The self-ability score was 3.33±0.40, and contribution from financial support was the smallest(4.09%). According to the set pair analysis, village doctors′ vulnerability total score was 0.49±0.06, ranging in a medium category. According to the sample cluster analysis, 27.2%(277/1 018)of the village doctors stayed at the upper category, as 30.3%(309/1 018) was defined as a medium category. According to the quadrant analysis, 35.5%(361/1 018)of the village doctors were found as in crisis vulnerability. Conclusions In general, village doctors′ vulnerability stays in the medium category. They have to fight against high disturbance from surroundings, with more emotional support and less financial support expected. 目的探讨医疗改革背景下山东省乡村医生脆弱性现状。方法采用自行设计的乡村医生调查表,于2015年10月至2015年11月进行横断面调查。问卷包括乡村医生的基本状态、环境干扰、工作干扰、情感支持、工具支持和自我能力6个部分。采用平均值和标准差来描述干扰和支持的水平。采用象限分析法对乡村医生的脆弱性进行分析。采用集对分析法计算脆弱性指数,采用样本聚类分析法根据脆弱性指数对乡村医生进行分类。结果干扰总分为3.39±0.46,职业风险贡献最大(19.95%),自我能力得分为3.33±0.40,经济支持贡献最小(4.09%)。根据样本聚类分析,27.2%(277/1 018)的乡村医生停留在较高类别,30.3%(309/1 2018)被定义为中等类别。根据象限分析,35.5%(361/1 018)的乡村医生被发现处于危机脆弱性中。结论乡村医生的脆弱性总体上处于中等水平。他们必须与来自周围环境的高度干扰作斗争,期望得到更多的情感支持和更少的经济支持。关键词:卫生人力资源;乡村医生;脆弱性;集对分析;山东省

Construction and implementation path analysis for precision medicine service system 精准医疗服务体系的构建与实施路径分析

Precision medicine can achieve precise prevention and treatment of diseases and improve medical standards. The precision medical service system is a carrier and medium for providing precision medical services, which can promote the implementation of precision medical plans and satisfactorily meet the medical service needs of patients. On the basis of literature research and practice, the authors studied the actual situation and development trend of precision medical development in China and built a general framework of precision medical service system. This framework is based on the precision medical service supply system and demand system, centering on the service product system. Ensured by the supporting system, they proposed the implementation path of precision medical service system construction. Online hospitals in China: existing problems and development suggestions 中国在线医院存在的问题及发展建议

The construction and development of online hospitals will maximize the use of existing health resources, improve the accessibility of high-quality medical care, and alleviate the difficulty in getting medical care for the masses. In this perspective, the authors briefly described the current construction and operation of online hospitals in China, and analyzed in detail the problems existing in the operation management, supporting policies among others. On such basis, they put forward suggestions from the perspectives of construction subjects and supervision subjects, so as to provide references for the benign development of online hospitals. Evaluation and analysis on the degree of patients′ trust in community TCM physicians under the TCM service capability promotion program 中医服务能力提升项目下社区中医医师患者信任度评价分析

Objective To evaluate the degree of trust of patients in community traditional Chinese medicine(TCM)physicians, based on the TCM service capability promotion program, and to analyze the impacts by TCM prevention and health care service. Methods 1 391 patients at 22 primary health service centers in 3 cities from Zhejiang province were investigated using the Chinese version of Wake Forest Physician Trust Scale(WFPTS)in September, 2017. Quantitative and qualitative variables were described by mean±SD, and ratio respectively. Differences were compared by t test, ANOVA and Kruskal-Wallis test. Results Chinese version of WFPTS had good reliability and validity in measuring community TCM physicians. The average scoring of patients′ trust in community TCM physicians was 38.00±6.42, while their benevolence and competence were 19.14±3.41, 18.82±3.50 respectively.For patients with different demographic characteristics, their quantity and the way to acquire and utilize TCM prevention and health care service had different degrees of trust in community TCM physicians. Patients′ gender, age, awareness and utilization of TCM prevention and health care service, etc. affect the degree of trust in community TCM physicians. Conclusions Patients have higher degree of trust in community TCM physicians, and improving the rate of awareness and utilization of TCM prevention has positive impacts on the degree of trust in these physicians. It is proposed that health knowledge education be strengthened and skills of community TCM physicians′ capacity be enhanced through various ways. 目的评价社区中医服务能力提升项目实施后患者对中医医师的信任程度,分析中医预防保健服务对社区中医医师的影响。方法采用中文版韦克森林医生信任量表(WFPTS)于2017年9月对浙江省3个城市22个基层卫生服务中心的1 391例患者进行调查。定量变量和定性变量分别用mean±SD和ratio描述。差异比较采用t检验、方差分析和Kruskal-Wallis检验。结果中文版WFPTS量表对社区中医医师具有良好的信效度。患者对社区中医医师的信任平均得分为38.00±6.42分,对社区中医医师的仁慈和胜任平均得分分别为19.14±3.41分和18.82±3.50分。不同人口特征的患者对社区中医医师的信任程度不同,其获得和利用中医预防保健服务的数量和方式也不同。患者的性别、年龄、对中医预防保健服务的认知和利用等因素影响着社区中医医师的信任度。结论患者对社区中医医师的信任度较高,提高中医预防知识知知率和使用率对社区中医医师的信任度有正向影响。建议通过多种途径加强健康知识教育,提高社区中医能力技能。关键词:社区卫生服务;卫生人力资源;中医内科医生;医患信任;预防和保健服务

Case study of different types of telemedicine diagnostic projects 不同类型远程医疗诊断项目的案例研究

Objective To compare the similarities and differences of four telemedicine diagnostic projects, for providing evidence-based policy-making advice for the development of telemedicine diagnostic projects in China. Methods Four projects led by the government, close medical alliances, loose medical alliances and companies were selected from November 2018 to June 2019. Through semi-structured interviews with different leaders, the authors analyzed the characteristics, problems and difficulties of different projects from three dimensions of organizational system, working conditions and operational effects. Results A total of 18 people were interviewed. Projects were different from each other in leaders, connection mechanism and effectiveness, participants and timeliness as well. The projects led by the government and close medical alliances had limited local participants with strong connection, clear but limited sources of diagnostic doctors and relatively large workload. While the projects led by loose medical alliances and companies had a wide range of participants, relatively weak connection efficiency and long response time. Conclusions Different types of telemedicine diagnostic projects have supplemented the manpower shortage of diagnosis physicians at primary medical institutions, introduced new service participants, as well as improved the medical service system. Projects were different from each other in leaders, connection mechanism and effectiveness, participants and timeliness as well. The projects led by the government and close medical alliances had limited local participants with strong connection, clear but limited sources of diagnostic doctors and relatively large workload. Analysis of main result quality indicators of tertiary maternal and child health hospitals 三级妇幼保健院主要结果质量指标分析

Objective To acquire the main result quality indicators status of tertiary maternal and child health(MCH) hospitals in China, supporting the decision making. Methods The National Maternal and Child Health Institutions Resources and Operations Survey Direct Reporting System was used to collect the management operation, inpatient death, re-return, hospital-acquired infection, surgical complications and patient safety of these hospitals in China in 2017. Statistical descriptions were made using average, rate and composition comparison data. Results The proportion of health technicians of tertiary MCH hospitals in China was 83.66%, the average hospitalization days were 5.96 days, with bed occupancy rate of 90.01%. The success rate of neonatal resuscitation was high. Meanwhile, the mortality rate of hospitalized maternal critical illness, the total hospitalization mortality rate of neonatal patients, the incidence of complications in surgical patients, the incidence of neonatal injury and the incidence of birth injury in vaginal delivery were higher in the institutions that had not participated in the MCH hospital accreditation. Conclusions The management of tertiary MCH hospitals in China was in good condition, and the relevant policies and projects have achieved remarkable results. Compared with general hospitals, hospital infection and re-return indicators were good. The quality and safety of tertiary MCH hospitals which have not participated in the MCH hospital assessment were poor. It is recommended to carry out MCH hospital accreditation to improve quality and safety. 目的了解我国三级妇幼保健院主要结果质量指标的现状,为决策提供依据。方法采用全国妇幼保健机构资源与运营调查直报系统,对2017年全国妇幼保健院的管理操作、住院死亡、复诊、医院获得性感染、手术并发症和患者安全情况进行统计。使用平均值、比率和成分比较数据进行统计描述。结果我国三级妇幼保健院卫生技术人员占83.66%,平均住院天数5.96天,床位占用率90.01%,新生儿复苏成功率高。同时,未参加妇幼保健院认证的机构住院孕产妇危重症死亡率、新生儿总住院死亡率、手术并发症发生率、新生儿损伤发生率和阴道分娩产伤发生率较高。结论我国三级妇幼保健医院管理状况良好,相关政策和项目实施效果显著。与综合医院相比,医院感染和再返岗指标良好。未参加妇幼保健医院评估的三级妇幼保健医院的质量和安全性较差。建议进行妇幼保健院认证,以提高质量和安全性。关键词:医疗保健调查;妇幼保健院;全面质量管理;结果质量指标

Research on influencing factors of precision medical service implementation based on clinician cognition 基于临床医生认知的精准医疗服务实施影响因素研究

Objective To investigate the influencing factors of the implementation of precision medical services based on clinician cognition, and provide a scientific reference for the implementation and advancement of precision medical services. Methods Using electronic questionnaires, clinicians from 48 hospitals in 12 provinces(autonomous regions and municipalities directly under the Central Government) were surveyed from July to September 2019, with 341 valid questionnaires collected. Descriptive statistics and factor analysis were performed on the survey data. Results Three dimensions that affect precision medical services were identified from the recovered questionnaires, namely external support factors, hospital drive capabilities, patient information provision and protection. The contribution rate of variance after orthogonal rotation was 35.157%, 22.234% and 16.343% respectively. Conclusions The external supporting factors should be developed, the hospital driving ability should be strengthened, and the patient information reception and privacy information security should be ensured to provide an important basis and guarantee for the implementation of precision medical service. Present situation of the staff of ultrasound departments in Shanghai medical institutions 上海市医疗机构超声科工作人员现状

Objective To investigate the current basic situation of the staff of ultrasound departments in Shanghai′s medical institutions, for providing references in making management policy of these professionals. Methods Questionnaire surveys on human resource and service ability were made to all the ultrasound departments of medical institutions in Shanghai in December 2013 and November 2018 respectively. Data of the two surveys were compared and analyzed, and were descriptively analyzed by mean and percentage. Results The number of ultrasound professionals per 10 000 people in Shanghai was 1.04 in 2018. Tertiary hospitals had advantages in the number of the professionals, and the proportion of professional qualification, age, education background and professional title of the professionals. Compared to those data in 2013, the number of ultrasound professionals had increased 31.8% in 2018. The proportion of medical practitioners with medical imaging specialty was 95.6%(2 063/2 158), and had increased by 4.7 percent. The medical services workload of ultrasound was 19.82 million person-time, and had increased 45.8%. Conclusions Development of ultrasound departments was rapid, but the development of professionals was unbalanced with the development of medical services. It is suggested to strengthen training of ultrasound professionals and improve the system of hierarchical medical system. Practical exploration of " Internet+ nursing service" for home care mode in Ningbo “互联网+护理服务”居家养老模式在宁波的实践探索

The " Internet+ Nursing Service" has been explored in localities as a new type of medical service. The paper introduced the innovation and excellence in the operation of Ningbo in the past four years, namely a unified operation platform to save resources, marked-based pricing to care for interests of three parties, nursing qualifications to safeguard quality of service, contracting with hospital entities to clarify responsibilities and rights, government leadership to determine assessment goals, and Ningbo Nursing Association in guidance to ensure homogeneous service. Thanks to the above measures, no disputes and complaints were recorded among 3 088 person-times ever since. Experiences in Ningbo may serve as a reference for such model to be implemented smoothly and orderly in other places. Key words: Nursing care; Internet+; Home care; Practice; Ningbo
