The innovative healthcare plus elderly care model is a major move for China to combat population aging. Given its initial success, there still exist challenges in the systems and economic conditions, for which family doctors can play a key role as population health " gatekeepers" . From the perspective of supply-side reform, this paper analyzed the current progress and challenges in this regard, and proposed a win-win mode. This mode combined the advantages of the family doctor system to optimize the community-based healthcare and elderly care as well as aging care resources.Family doctors are centered to build a community elderly care supply mode, to promote mutual development of primary medical institutions and the elderly-healthcare sector in China. Objective To understand the healthcare and elderly care needs of the elderlies. Methods From July to December 2018, a questionnaire survey was conducted to investigate the healthcare and elderly care status and characteristics of the elderly under different modes of combination of medical care and elderly care, and to analyze their objective needs for both cares. Results There was an outstanding need for daily life care for the elderlies in the healthcare plus elderly care mode. 71.8% of them were senior elderlies, and 61.1% had obvious disabilities. It was found that elderly-adaptive transformation was required for their living environment in the home care mode, as 41.7% of them live in elevator-free buildings, and 48.2% use squat-type toilets. The elderlies need in general cultural services, and widowhood and solitary living were common. The home care mode required disease prevention services, as 43.6% of them had weight problems, and smoking and drinking rates were very high(9.4%, 13.7%). The elderly care in medical mode required medical services, because the proportion of two or more chronic diseases was very high(27.2%). Conclusions The healthcare and elderly care modes vary in terms of needs, thus it is necessary to adapt to specific conditions. Effective communication between doctors and patients can reduce the occurrence of medical complaints and disputes. The office of doctor-patient relationship acts as the complaints management department of the medical institution and undertakes the management of medical complaints. Since 2017, our hospital has introduced the staff of the doctor-patient relationship office to participate in major preoperative conversations. The mode establishes a communication bridge between clinicians and surgical patients and their families, so that patients′ informed consent is more complete and more realistic, and the sufficiency and effectiveness of doctor-patient communication is enhanced. It has a positive effect on medical complaints and dispute risk prevention. Objective To construct and implement a calculation model of clinical nursing post classification. Methods Between October to December 2018, head nurses of the hospital were screened as consultants, and Delphi method was used to determine the indicators, while the weights were assigned from the aspects of nursing workload, work quality, patient satisfaction and number of nursing night shifts. Combined with HIS data, the calculation model of clinical nursing post classification was constructed to classify clinical nursing units into different categories and levels. Results After two rounds of expert enquiry, 82 nursing work items were identified and the objective weight assignment was determined ranging from 0.80 to16.14. According to the established calculation model and HIS data, nursing posts in clinical departments were classified into 6 levels and 3 categories, and the accurate management of clinical nursing post classification was achieved. Conclusions The construction of a calculation model is scientific and rigorous, which provides a scientific basis for dynamic nurse performance management and rational allocation of human resources. Post-payment for patients can minimize their errands and improve their medical experience. Since February 2019, Hangzhou Municipal Health Commission began to try the " post-payment" service. This mode is integrated into the municipal credit system, and the patients can obtain the corresponding credit line according to their credit rating. Within such line, the patient can pay only once for all their medical services received per visit. The mode covers all Hangzhou residents with credit rating and has served nearly 20 million outpatient and emergency patients with expedient services. Thanks to the reform, the number of payment for patients to see a doctor reduced from an average of 2 to 3 times to only 1 time. in view of the emphasis on disease management and clinical research, the authors introduced the evaluation and construction methods of the departments based on disease management. Introduction to the selection process of the evaluation and construction methods of such departments, the authors expounded the construction thought of clinical research evaluation system. This paper analyzed the important role of clinical scientific research in the formation of hospital characteristic advantage and leading position, then suggested that hospitals should take research-type diseases as the basis, promote steady breakthroughs in their medical services and scientific research level, and fully reflect the responsibilities of tertiary hospitals. In order to fully understand the status quo of the healthcare plus elderly care mode development in China, this paper comprehensively analyzed the progress of China in this respect, from four aspects as follows. These include the policy making, service mode development, number of elderly care beds and long-term care insurance. In view of the current shortage of resources in China, namely that of talents, poor teambuilding, imbalance between supply and demand of medical institutions, and the lack of coordination among healthcare and elderly care institutions, this paper suggests that China should focus on strengthening the construction of diversified elderly care system and professional elderly care personnel. In addition, it suggests to establish and improve the long-term care system with Chinese characteristics and promote the introduction of hospice care services for elderly care institutions, hence encouraging a high level healthcare plus elderly care career in China. Objective To analyze the influencing factors of hospitalization cost by back propagation(BP)neural network. Methods Inpatients with a total of 50 611 were collected from the hospital information system in a tertiary hospital. BP Neural network modeling was developed by SPSS 22.0 software to study the factors which influence the hospitalization cost. Results The hospitalization expenses prediction accuracies of training sample and test sample were 81.1% and 79.0% respectively. The top three factors were pharmaceutical cost(100.0%), general medical operation cost(88.8%)and disposable medical materials cost(60.7%). Conclusions The results show that BP model is suitable for dependent variable with the hospitalization expenses. The pharmaceutical cost, disposable medical materials cost and general medical operation cost were greater influencing factors than others. Objective To analyze the equity of the performance appraisal model of primary health care institutions based on the relative value index. Methods Tian Tan Community Health Service Center in Dong Cheng District of Beijing was taken as an example, while descriptive analysis and correlation analysis were conducted using the data of service equivalent per capita and monthly performance income per capita of 14 departments from April 2018 to June 2019. Results In the first half of 2019, the average monthly work equivalent of the case institution was 1 170.19±501.18, which was 13.91% higher than that of the second quarter in 2018. The average monthly performance income of the case institution was 1 183.71±175.30 Yuan, which was 6.94% higher than that of the second quarter in 2018.From April 2018 to June 2019, the monthly work equivalent per capita of 14 departments was positively correlated with the monthly performance income per capita, and Pearson correlation coefficient was 0.85(P<0.01). Conclusions The performance appraisal model based on the relative value index in case institution satisfactorily reflects the fairness of " more work and more gain" , but the effect of " better work and better gain" is not obvious.
