La précarité a été associée à une augmentation de la mortalité liée au cancer du sein mais les liens entre précarité, stade au diagnostic et parcours de soins sont peu explorés. L’objectif de l’étude DESSEIN était d’évaluer l’impact de la précarité sur la maladie et les parcours de soins.

Au total, 875 patientes ont été incluses entre 2016 et 2019 dans dix-neuf établissements : 543 patientes non précaires et 332 patientes précaires. Les patientes précaires avaient un stade au diagnostic plus avancé (55 % de T1 versus 63 %, 30 % de N+ versus 19 %, p = 0,0006), avaient un risque plus élevé de ne pas recevoir le traitement initialement prévu (4 versus 1 %, p = 0,004), et participaient moins à des essais cliniques (5 versus 9 %, p = 0,03). Le non-recours aux soins oncologiques de support était deux fois plus fréquent chez les patientes précaires (p < 0,001). En cours de traitement, 33 % des patientes précaires déclaraient subir une perte de revenus contre 24 % des patientes non précaires (p < 0,001). À douze mois du diagnostic, les licenciements étaient deux fois plus nombreux chez les patientes précaires (p = 0,0001).
La précarité affecte toutes les étapes de l’histoire du cancer et du parcours de soins. Une attention particulière doit être portée aux populations vulnérables, prenant en compte les questions d’accessibilité géographique et financière, la littératie en santé et les biais implicites des soignants.

La prise en charge de la douleur multimorphe du cancer est un soin de support majeur en oncologie, pour lequel de nombreuses recommandations nationales et internationales ont été récemment actualisées. Tout patient atteint d’un cancer en France doit bénéficier de l’accès aux soins de support et ce dès le diagnostic de cancer, tout au long du parcours de soins. L’insuffisance rénale chronique, qu’elle préexiste ou non au cancer et à ses traitements, nécessite de la part des équipes interdisciplinaires soignantes une attention toute particulière et un souci constant du détail pour proposer la meilleure stratégie thérapeutique antalgique, associant les approches complémentaires et interventionnelles aux traitements médicamenteux. Les médicaments antalgiques, en particulier les opioïdes et les antineuropathiques, répondent à des précautions d’usage qu’il faut prendre en compte au quotidien. Une alliance thérapeutique intégrant la pharmacie clinique en tant que soins de support à part entière est un atout majeur permettant d’optimiser et de sécuriser les traitements médicamenteux antalgiques, conditionnant à la fois leur efficacité, leur tolérance et l’observance thérapeutique, dans l’objectif d’améliorer la qualité de vie du patient.
The management of multimorphic cancer pain is a major supportive care in oncology, for which many national and international recommendations have recently been updated. Any cancer patient must benefit from access to supportive care from the diagnosis, throughout the entire care pathway. Chronic renal failure, from any etiology, requires special attention and constant attention to details from interdisciplinary caregivers’ teams to propose the best analgesic therapeutic strategy, combining complementary and interventional approaches to treatments. Analgesic therapies, in particular opioids and antineuropathics, require specific precautions. A therapeutic alliance integrating clinical pharmacy as a supportive care in its own right, is a major asset allowing the optimization and securing of analgesic drug treatments, conditioning both their efficacy, their tolerance and therapeutic observance, in the goal of improving the patient's quality of life.
A better understanding of the RNA biology and chemistry is necessary to then develop new RNA therapeutic strategies. This review is the synthesis of a series of conferences that took place during the 6th international course on post-transcriptional gene regulation at Institut Curie. This year, the course made a special focus on RNA chemistry.
La dysfonction rénale aiguë et chronique est fréquente au cours du myélome multiple et entraîne une morbi-mortalité importante. La néphropathie à cylindres myélomateux, par précipitation intra-tubulaire des chaînes légères libres monoclonales avec l’uromoduline, en est la principale cause. La biopsie rénale, bien que non nécessaire au diagnostic, peut aider à évaluer le pronostic de récupération rénale. La prévention et le diagnostic précoce de l’atteinte rénale sont primordiaux afin d’optimiser la prise en charge et d’éviter l’évolution vers une insuffisance rénale chronique. La réhydratation, l’arrêt des traitements néphrotoxiques, la correction des facteurs de risque de précipitation et la chimiothérapie anti-plasmocytaire permettent de réduire rapidement la néphrotoxicité des chaînes légères libres. L’association de l’inhibiteur du protéasome bortezomib à la dexaméthasone haute dose est le traitement de référence au diagnostic d’un myélome multiple avec atteinte rénale. L’ajout du cyclophosphamide ou de l’immunomodulateur lenalidomide peut améliorer l’efficacité hématologique, mais au prix d’une moins bonne tolérance. L’ajout d’anticorps monoclonaux anti-CD38 est en cours d’évaluation dans cette population. L’hémodialyse avec des membranes à haut-flux ou avec des membranes à haut point de coupure, associée à la chimiothérapie, peut participer à améliorer la récupération de la fonction rénale. La prise en charge du myélome multiple doit s’adapter chez les patients insuffisants rénaux chroniques, dialysés ou transplantés rénaux. En raison d’une amélioration de la survie globale, la transplantation rénale reste une option à considérer chez certains patients. La collaboration entre hématologues et néphrologues est essentielle tout au long de l’évolution de la maladie.
Renal impairment is common during multiple myeloma and persistent reduction in kidney function strongly affects prognosis. Cast nephropathy, by monoclonal free light chains precipitation with uromodulin in renal tubules, is the main cause of acute kidney injury in multiple myeloma. Kidney biopsy, although not necessary for diagnosis, allows assessment of renal prognosis according to the extent of cast formation, tubular atrophy and interstitial fibrosis. Prevention and early diagnosis of acute kidney injury are essential to optimize management and avoid progression to chronic kidney disease. Rehydration, interruption of nephrotoxic treatments, correction of precipitating factors, anti-plasma cell chemotherapy can rapidly reduce the free light chains nephrotoxicity. The association of the proteasome inhibitor Bortezomib and high dose Dexamethasone is the reference treatment in newly diagnosed patients with renal impairment. Adding Cyclophosphamide or the immunomodulator Lenalidomide may improve the hematological response, but with a poorer tolerance. La prise en charge d’une tumeur rénale maligne impose tout d’abord une réflexion sur l’opportunité de son traitement, en prenant en compte la fonction rénale, d’autant plus si cette dernière est altérée au moment du diagnostic, ce qui est le cas chez 50 % des patients. Le choix de la méthode de traitement dépend de nombreux facteurs, et en particulier de la fonction rénale résiduelle prévisible, du risque de maladie rénale chronique, du besoin de recours temporaire ou à long terme de la dialyse, et enfin de la survie globale à long terme. D’autres facteurs interviennent comme la taille, la position et le nombre des tumeurs, ainsi que l’existence d’un contexte de tumeur héréditaire. La néphrectomie totale ne doit plus être réalisée chez les patients présentant des petites masses rénales malignes (cT1a) lorsqu’il existe une alternative possible de prise en charge épargnant la fonction rénale. Elle peut consister en une chirurgie (néphrectomie partielle ou tumorectomie), une thermo-ablation percutanée (par radiofréquence, micro-ondes ou cryothérapie). Chez les patients dont l’espérance de vie est limitée, une surveillance basée sur l’imagerie peut être proposée, afin de proposer un traitement en cas de progression locale. La bonne coordination entre urologue, radiologue, néphrologue et parfois radiothérapeute doit permettre une prise en charge optimale des patients atteints d’une tumeur rénale maligne avec ou sans insuffisance rénale sous-jacente.
Managing a malignant renal tumor requires, first of all, a reflection on the necessity of its treatment. It must consider the renal function, altered at the time of diagnosis in 50% of cases. The treatment method chosen depends on many factors, in particular, the predicted residual renal function, the risk of chronic kidney disease, the need for temporary or long-term dialysis, and overall long-term survival. Other factors include the size, position, and number of tumors and a hereditary tumor background. When a renal-sparing management alternative is available, total nephrectomy should no longer be performed in patients with small malignant renal masses (cT1a). This may consist of surgery (partial nephrectomy or lumpectomy), percutaneous thermo-ablation (by radiofrequency, microwave, or cryotherapy). In patients with limited life expectancy, imaging-based surveillance may be proposed to suggest treatment in case of local progression. Good coordination between urologist, radiologist, nephrologist, and sometimes radiotherapist should allow optimal management of patients with a malignant renal tumor with or without underlying renal failure.
