Background: Platelet distribution width and mean platelet volume are markers of platelet activation. Platelets are not only involved in hemostasis and coagulation, but play a signicant role in innate immunity and inammatory response. Excess production of cytokines and acute phase reactants affect megakaryopoiesis resulting in the release of immature platelets from the bone marrow altering platelet indices. In this study we aimed to investigate the relationship of platelet indices in RTPCR proven COVID-19 patients and non COVID patients. Methods: An observational comparative two group cohort clinical study was conducted on 200 COVID-19 patients and 100 normal individuals. The platelet indices like Platelet count, Mean platelet volume (MPV), Platelet distribution width (PDW), Platelet large cell ratio (P-LCR) and Plateletcrit (PCT) were compared among two groups. Results: Platelet count in COVID-19 patients were low (p<0.001) compared to controls. P-LCR was also signicantly decreased in COVID-19 patients in comparison to non-COVID individuals. MPV, PDW and PCT were high in COVID 19 patients in comparison to controls, but was not signicantly raised in a large number of cases. Conclusion: Platelet indices like platelet count, MPV, PDW, PLCR and PCTare simple, cost effective and easily available laboratory parameters from routinely used automated hematology analyzers which are signicantly altered in COVID-19 infection and thereby can be used as biomarkers in COVID-19.

Background: Diabetic foot ulcers (DFUs) are serious consequence of poorly controlled diabetes. DFUs are a therapeutic challenge in developing countries. This observational study aimed to analyse surgical characteristics, management modalities and short-term treatment outcomes of DFUs. Methods: This study was conducted on 155 patients. Data covering detailed history, management modalities and short-term outcomes were collected through an interviewer-administered structured questionnaire. Results: Mean age of patients is 58.32 ± 14.24 years, comprising 57.4% males. 54.8% patients belong in upper-lower class. 51.6% had diabetes for 5-10 years, 54.8% with very poor HbA1C > 10%. Ulcers involving the toes 34.8% and 82.6% ulcers were > 3 cm². Severe DFUs (Wagner grade 3-5) were seen in 75.4% of patients. Main intervention was surgical debridement. 37.4% suffered from SSI; revision amputation was done in 11.6% patients. Mean length of hospital stay 19 ± 12.1 days and mortality rate was 1.33%. Conclusion: Majority of patients had advanced DFUs. Larger prospective study should be done for proper management. Screening and proper evaluation can reduce complications.

Current literature on physical and mental health of a surgeon in India is scarce. We conducted a questionnaire-based cross-sectional, descriptive study across surgical trainees and consultants and studied multiple aspects of the surgeons' physical and mental well-being. 75.72% (262/346) of surgeons reported experiencing work-related musculoskeletal disorders of varying degrees as a direct result of their work. Moreover, out of 346 surgeons, 66% (228 surgeons) reported that they feel emotionally exhausted after work, several times a month or every day. 33% (114 surgeons) admitted to having no feelings or an impersonal response towards patients and 50% (173 surgeons) had a low sense of personal accomplishments. It is suggested that surgeons become aware of their state of well-being, given the inevitability of physical and mental fatigue in a surgical career, and take measures to improvise their quality of life.

Purpose: To analyse applications of blindness certicate to nd out causes of blindness in tertiary eyecare Institute of Ophthalmology in western India. Methods: Data was collected from patients coming to tertiary eyecare Institute of Ophthalmology in western india during the course of 2 years from 2019 to 2021. Patients were explained about the procedure and written consent was taken. After thorough examination, nal diagnosis was made.. In this study patients with blindness impairment > 40% who were entitled to government benets were taken. All patients were analysed and categorised according to age group and aetiology. The visual impairment percentage was given to them by using disability percentage and denition proposed by International Classification of Disease 11 (ICD 11, 2018) Results: and then compared with other studies. 200 patients who came for blindness certicate at our institute were included in our study. Out of this, 29% belonged to 46-60 year age group. 136 patients (68%) were found to be 100% visually disabled and eligible for all benets and concessions. Retinal diseases were the commonest cause for visual disability in the age group between 46-75 years (23%). Retinal diseases were the commonest cause fo Conclusion: r visual disability in elders. In age group 0-15 year, retinal diseases and retinitis pigmentosa were the main reasons for visual disability. These type of studies help in population based surveys to plan, implement and supervise eye care health services.

The present study is an attempt to prepare Rasanjan and evaluate the quality control parameters by doing analytical study.

Blunt Cardiac Injuries are injuries acquired due to blunt trauma to the heart. Minor ECG or cardiac enzyme abnormalities and arrhythmias are a subset of BCI, referred to as cardiac contusions. BCI is associated with signicant morbidity and mortality, and due to the absence of a denitive diagnostic test, is difcult to diagnose. Delayed presentation of arrhythmias beyond the initial 48 hours of trauma is rare.We present a case of an elderly female, who presented with atrial brillation ten days after sustaining a blunt chest trauma. She was managed in accordance with the AHA ACLS guidelines, but ultimately succumbed within an hour of Emergency stay. Elderly patients may require prolonged monitoring after blunt chest trauma.

Radical resection of renal cell carcinoma with renal vein & supradiaphragmatic inferior vena cava thrombosis, is often associated with signicant amount of blood loss during the surgery, owing to the dense peri-renal reactive adhesions, altered hilar anatomy & parasitic/collateral vessels from the surface of kidney to colon and retrohepatic area & retroperitoneum. It is standard practice to proceed with nephrectomy initially, followed by retrieval of the thrombus; however, the opposite can also be performed. Invariably, surgery in these cases is associated with signicant blood loss, mainly due to the extensive network of peri-nephric collateral vessels. Hence, adequate preoperative patient preparedness is the norm. In this case series, we aim to highlight a novel method of decreasing the intra-operative time & blood loss associated with nephrectomy, during radical resection of renal tumours associated with tumour thrombus which eventually led to decreased intra-operative blood loss and operative times.

The incidence of benign renal tumours is less, especially when compared to renal cell carcinomas, as most are detected incidentally or at autopsy.1 Reported in 7% of kidneys resected for other tumors, they are more frequent in resected kidneys accompanying RCC, papillary type compared with other subtypes.2 Present in 7 - 40% of autopsy studies, frequency increases with age.2 We present a case of 30 year old deceased male whose viscera including brain, heart, lungs, liver, spleen and kidney were received in the Department of Pathology, PGIMS, Rohtak, Haryana for histopathological examination. The histopathological examination of the kidney showed Renal papillary Adenoma.

A 65 year old male patient was admitted in our hospital with complaints of breathlessness on exertion, haematemesis, history of malaena. Patient had history of loose stools 10 days back and was on tab metronidazole 400mg tid for 5 days. On general examination, there was pallor and laboratory investigations showed PT ( test ) more than 100, peripheral smear showed normocytic normochromic RBCs admixed with microcytic and macrocytic RBCs with anisopoikilocytosis showing occasional tear drop cells. Patient was on Tab Acitrom 2mg 0-0-2. Tab acitrom was stopped and patient was given Vit K, Fresh frozen plasma and Packed red blood cells transfusion. Patient's Haemoglobin started improving and PT came within normal limits. Patient improved symptomatically

On general examination, there was pallor and\u0000laboratory investigations showed PT ( test ) more than 100, peripheral smear showed normocytic normochromic RBCs admixed with microcytic\u0000and macrocytic RBCs with anisopoikilocytosis showing occasional tear drop cells. Patient was on Tab Acitrom 2mg 0-0-2. Tab acitrom was\u0000stopped and patient was given Vit K, Fresh frozen plasma and Packed red blood cells transfusion. Patient's Haemoglobin started improving and PT\u0000came within normal limits. This paper explores the increasing need for trauma informed approach to juvenile rehabilitation and Juvenile justice system. Drawing upon research ndings, empirical evidence and theoretical framework, it aims to study the indomitable relationship between trauma and delinquency recognizing the profound effect of trauma on youth offenders. It highlights the result of lack of trauma based approaches to rehabilitation among juvenile offenders. By exploring collaborative intervention, trauma based practices, ripple effect of trauma and the prevalence of trauma among young offenders, this paper underscores the importance of adopting a trauma based lens while supporting impacted youth, facilitating their rehabilitation and integration into society. In summary, it calls for a shift away from punitive punishment towards making amends for victims' rights while at the same time advocating for healing, safety as well as accountability. Lastly, it argues for a multidisciplinary approach.
