Zenker's diverticulum is a rare disorder characterized by the herniation of the post-pharyngeal wall through the Killian's triangle, located between the inferior constrictor muscle and the cricopharyngeus muscle. Surgical intervention used to be the primary treatment for Zenker's diverticulum, despite its high morbidity and mortality rates. In recent years, Z-POEM, a technique utilizing third-space endoscopy, has emerged, specically tailored for Zenker's diverticulum. It is a relatively new addition to the natural orice transluminal endoscopic surgeries (NOTES). Usually carried out in endoscopy suites, a crucial step in the procedure involves creating a submucosal tunnel in the esophageal wall and the continuous insufation of CO2 can inadvertently lead to complications such as subcutaneous emphysema, capnomediastinum, capnothorax, or even capnoperitoneum, due to tracking of CO2 in surrounding tissues. Anesthesiologists face the challenge of not only administering anesthesia in a remote location, but also promptly detecting and managing these complications. Here, we outline the effective anesthetic management in the case of an adult male diagnosed with Zenker's Diverticulum who underwent Z-POEM procedure under General Anesthesia.

Aim: This study investigates the association of dry eye disease (DED) with dyslipidemia. The study also assessed the association of visual display terminal (VDT) and DED in patients with dyslipidemia. The meibomian gland dysfunction (MGD) in DED in patients with dyslipidemia was also studied. In the OPD, 60 patients aged 20-60 years with dyslipidemia were studied. Serum lipid levels were analyzed to establish the dyslipidemia-DED association. Sebaceoma is a benign adnexal neoplasm showing sebaceous differentiation. This report details a case of a 41-year-old male presented to OPD with complaints of a raised lesion over scalp for 6 months. Excision biopsy was done and sent to our histopathology lab for denitive diagnosis by the clinician with the differential diagnosis of solitary trichoepithelioma. Histopathology showed mature sebocytes and greater than 50% of the tumour cells were basalaoid cells which is consistent with sebaceoma. Sebaceomas are rare skin adnexal neoplasms. It may be associated with Muir-Torre syndrome. It is characterised by multiple rounded cellular lobules and microcystic spaces predominantly composed of immature sebocytes admixed with mature sebocytes.

Mucocele of the appendix occurs due to obstruction of lumen of appendix resulting in abnormal accumulation of mucous in the lumen, giving the appendix an enlarged appearance with thinned out wall. Preoperative clinical diagnosis may be challenging due to non-specic clinical manifestations of the disease or may mimic acute appendicitis. Radiological imaging is essential for diagnosis. When diagnosed preoperatively, it is always treated by surgery which depends on size and integrity of the appendix base and on the histology of the lesion. During surgery, care should be taken to avoid spillage of its contents into the abdomen which may lead to pseudomyxoma peritonei (PP), which has very poor prognosis.

Introduction: The anterior cruciate ligament (ACL) is a critical structure within the knee joint, playing a pivotal role in maintaining stability during various dynamic movements, including cutting, pivoting, and jumping. This study aims to evaluate the predictive value of pre-operative static ATT in knees with ACL injury. Material and Methods: This study constituted a retrospective descriptive research endeavour encompassing patients who underwent both MRI and knee arthroscopy at Jaipur National University, Institute for Medical Science and Research Centre from October 2023 to March 2024. Clinical data gleaned from in-patient records from the Department of Orthopaedics included demographic particulars such as age, sex, height, weight, and occupation. Patients were dichotomized into two subgroups predicated on arthroscopic ndings: partial ACL tear or complete ACL tear. Subsequently, patient diagnostic images were reviewed, and anterior tibial translation distances were meticulously measured. The means of anterior tibial translation distances in each subgroup were then computed, and the unpaired t-test for the mean difference of each subgroup was executed. Furthermore, a comparative analysis was conducted between the cut-off value derived in this investigation and those reported in antecedent studies. MRI protocols entailed knee protocol scans with the 1.5-Tesla Siemens Sempra, encompassing sagittal, coronal, and axial planes with distinct pulse sequences. Anterior tibial translation distance was meticulously dened as the distance between the most posterior cortex of the tibia and femur, acquired at the midsagittal plane of the lateral femoral condyle. The measurement was conducted relative to a plane parallel to the long axis of the image. Results: The results suggest that the test demonstrates outstanding sensitivity, specicity, positive predictive value, and negative predictive value, all reaching 100% (with condence intervals ranging from 84.56% to 100% for sensitivity and positive predictive value, and from 75.29% to 100% for specicity and negative predictive value). Conclusion: Employing a 7mm threshold on the anterior tibial translation sign for the diagnosis of complete ACL tears represents a valuable clinical tool with signicant implications for patient management.

Background: COPD is one of the major causes of chronic morbidity and mortality worldwide. It is the fourth leading cause of death in the world(1). Chronic airow limitation of COPD is caused by a mixture of small airway disease (obstructive bronchiolitis) and parenchymal destruction (emphysema), the relative contributions of which vary from person to person. Aim and Objective of the study: To Study the changes of Arterial blood gases in COPD phenotypes. Methods: This Descriptive study was carried out on the patients with chronic obstructive pulmonary disease who were admitted to Victoria hospital, Bangalore from AUGUST 2021 to OCTOBER 2021. Arterial blood gas analysis was used as a diagnostic test in 110 cases of Chronic Obstructive Pulmonary Disease. Interpretation and Conclusion: ABG sampling constitutes a more precise measure of successful gas exchange, oxygenation and happens to be only reliable determination of ventilation success as reected by CO2 content . It also provides valuable information on the acid base balance at a specic point in the course of a patient's illness and plays an important role in the management of Chronic Obstructive Pulmonary Disease. Patients regularly treated in phases of Remission and Exacerbations of COPD the course of illness is slower. Decrease of pH and PaO2 and increase of PaCO2 is statistically signicantly smaller in those received regular treatment in phases of remissions. ABG analysis is indicated in acute exacerbations, during assisted ventilation and to decide prognosis. It should become an integral part of patient management.

Background: Methods: Epidermal inclusion cysts are the most common cutaneous cysts, but malignant transformation in them is extremely rare. We present a case of a 63-year-old female with a swelling in the right gluteal region with associated discharge that was resected based on the clinical impression and cytology of an epidermal inclusion cyst. Histopathologic exam showed atypical cells with enlarged nuclei and prominent nucleoli with brisk mitotic activity, and on IHC, these cells were positive for PanCK and P40 and negative for Sox10, conrming the diagnosis of Poorly Differentiated Squamous Cell Carcinoma. There are 56 reported cases of SCC arising on a background Conclusion: of epidermal inclusion cysts in literature. Although epidermal inclusion cysts are usually benign, excision is recommended for atypical cysts with high suspicion for malignancy. They should be excised intact and should undergo histopathological examination to rule out incipient malignancy.

Postoperative sore throat (POST) is a common undesirable outcome of tracheal intubation, with prevalence ranging from 21% to 65%. This is inuenced by factors such as mechanical trauma during intubation, tracheal tube cuff pressure causing mucosal erosion, inammation, and dehydration [1,2]. Prolonged intubation can lead to serious consequences, while even a short tracheal intubation period of one hour can cause extensive damage to the tracheal and laryngeal epithelium. Although POST is usually a self-limiting condition that resolves within 12-24 hours, its prevention is important to reduce [3] hospital stay and improve patient satisfaction.

Objective: The objective of this systematic review is to compare and clinically evaluate the adjunctive benets of a diode laser (DL) to conventional open ap debridement (OFD) in the treatment of chronic periodontitis. DL is an excelle Background: nt soft tissue surgical laser, indicated for soft tissue curettage or sulcular debridement with an additional bactericidal effect. Studies have been conducted to determine the effect of DL as an adjunct to OFD. There is limited documentation regarding the clinical effectiveness to prove adjunctive effect of OFD in the surgical periodontal therapy. Only randomized controlled trials (RCTs) assessing use of DL only in Methods: continuous and contact mode on undersurface of ap post-conventional OFD for treatment of chronic periodontitis in the test group were considered. Studies published only in English language with follow-up periods of 3 and 6 months post-operatively and assessing probing pocket depth (PPD) and clinical attatchment level (CAL) as the outcomes were considered. A total of 6 RCTs were included in this systematic review. Cochrane Risk of Bias tool was used for assessing the risk of bias. 3/6 studies concluded that DL signicantly led to reduction in PPD or ga Results: in in CAL when used as adjunct to OFD. Whereas, other 3/6 studies concluded that DL did not signicantly lead to reduction in PPD or gain in CAL when used as adjunct to OFD. Conclusion: DL can be used as an adjunct to standard treatments (OFD) of chronic periodontitis i.e; chronic periodontitis rather than as a replacement for standard treatments.
