The recommendations of the “Haute Autorité de santé” about the reeducation and rehabilitation of the motor function of the musculoskeletal system of people diagnosed with cerebral palsy published in December 2021 were awaited by all professionals interested in this pathology as well as a large number of families. They lead us to reflect on the place of professional training, the message it conveys and its necessary evolution in terms of content and teaching methods. The central question is to wonder about the representation of the competent professional that we wish to target.

For the first time, the High Authority for Health publishes recommendations for good practice in motor rehabilitation for people diagnosed with cerebral palsy. A simple kick in the anthill or a real helping hand for families? Mum of Léonie, eight years old, suffering from cerebral palsy following a stroke in utero, I bring here my testimony and share my opinion on the subject. Too many families face a lack of listening and knowledge on the ground, which hinders access to rehabilitation favourable to the needs of their children. For lack of sufficient proposals from caregivers, many parents have made the same choice as me: take charge of the rehabilitation of their children sometimes against the advice of health professionals. It is urgent that families no longer have to fight to obtain the best rehabilitation for their children! The recommendations of the High Authority for Health have been published for this purpose. They formalize the crucial need for appropriate rehabilitation and rapid implementation after diagnosis. However, if they recommend certain methods and value the intensive, they come up against an obvious lack of scientific studies which does not always allow them to bring sufficiently refined conclusions. Finally, for families and therapists to find themselves united in the field of rehabilitation, the context of care for cerebral palsy must absolutely evolve and medical research must develop. The recommendations call for new projects!

The recommendations “Haute Autorité de Santé” relating to the rehabilitation of the motor function of the musculoskeletal system of people with cerebral palsy published in October 2021 provide professionals, as well as people with cerebral palsy and their families, precise information on the state of the art of the practices usually carried out. However, the reader of this recommendations should not take them for granted. Careful consideration should be given to the methodology used, as to the scientific data available. The recommendations therefore constitute a reference, at a given time, offering a general framework of good practices and very targeted recommendations. Everybody i.e., people with cerebral palsy, families, professionals must now seize the opportunity of these publications to improve their practices, to strengthen their training. They should be used to determined research subjects in order to enlighten many sectors of the rehabilitation sectors that are today hardly studied.

December 6th, 2021 marks an important milestone in the rehabilitation/rehabilitation of people with Cerebral Palsy, their loved ones and all rehabilitation/rehabilitation professionals. It was on this day that the HAS published the recommendations of good practices of “rehabilitation and rehabilitation of the motor function of the musculoskeletal system of people diagnosed with Cerebral Palsy”. These recommendations are the first in cerebral palsy. They represent a roadmap and a major paradigm shift. The recommendations allow, first of all, all rehabilitators to have a common base on the minimum expectation for the care of patients. They also allow everyone to have an identical communication tool. The other originality of these recommendations is to consecrate the co-construction of concrete rehabilitation objectives with the patient/his family and the rehabilitators. These recommendations remain theoretical. From there, the key importance of the Therapeutic Education of the Patient and the expert patient to help everyone to seize them and then to make them live.

The conclusions of the recommendations HAS according to Reeducation Techniques Le Métayer do not leave indifferent many professionals working with people with Cerebral Palsy. However, most of its teaching is found in the main principles proposed by HAS. There is no need to oppose these two approaches, clinical on the one hand and scientific on the other, but rather to seek the links that unite them in order to improve the quality of reeducation and rehabilitation care.

Cerebral palsy is the main cause of disability in children. It leads to motor, postural and/or sensory dysfunctions for which care is required from birth. This care must be diversified in order to avoid monotony and to maintain good adherence to care. Virtual reality, which has been in full expansion for several years, appears to be an interesting tool for maintaining motivation and diversifying the exercises and modalities offered in rehabilitation to children with cerebral palsy.
Method
A bibliographical research carried out in different search engines (CINAHL, PubMed) with the addition of filters allowed the selection of 21 articles. Five of these articles were selected for their close link with the key words and analyzed in the article below.
Discussion
From the upper limb to balance, virtual reality seems to have an impact on many areas of rehabilitation management. If it appears interesting on the motivation for all the authors, its use meets certain criticisms and the interpretation of the results must be taken with parsimony.
Conclusion
Virtual reality seems to be an interesting tool to use for the rehabilitation of children with cerebral palsy. However, other studies are necessary with less heterogeneity, the use of adapted and validated tools for cerebral palsy and a higher level of proof.
