A rare case of autoimmune dysglycemia syndrome associated with systemic lupus erythematosus and dermatomyositis 一例罕见的自身免疫性血糖异常综合征合并系统性红斑狼疮和皮肌炎

Autoimmune dysglycemia syndrome (ADS) is a rare condition that presents as episodes of hypoglycemia as well as hyperglycemia and is classified as insulin autoimmune syndrome (IAS) and type B insulin resistance (TBIR). Autoimmunity plays a key role in the pathogenesis of this disorder, as evidenced by the presence of autoantibodies against endogenous insulin or the insulin receptor, and by its association with rheumatologic disorders. Treatment usually includes glycemic control and immunomodulatory agents. We report a case of a 31-year-old woman who was admitted for severe hypoglycemia. Further workup revealed underlying systemic lupus erythematosus (SLE) with renal involvement. During hospitalization, she continued to experience episodes of fasting hypoglycemia, as well as episodes of postprandial hyperglycemia. Hypoglycemia associated with a high serum insulin concentration and positive anti-insulin antibodies were consistent with IAS. Likewise, hyperglycemia and hypoglycemia in association with weight loss, acanthosis nigricans, polycystic ovarian syndrome, and normotriglyceridemia strongly suggested TBIR, although testing for antibodies against the insulin receptor was not available in Peru. Immunosuppressive therapy and metformin were indicated, resulting in remission of SLE and ADS. Years later, the patient exhibited features of dermatomyositis, such as Raynaud’s phenomenon, muscular weakness, heliotrope exanthema, and elevated muscle enzymes. Once again, the patient received immunosuppressive therapy. ADS is an infrequent cause of hypoglycemia, and the coexistence of its two pathophysiological mechanisms in a patient with SLE and subsequent development of dermatomyositis is even more rare. Our case is the first one reported describing this association.

Artificial intelligence and improvement of stem cell delivery in healthcare 人工智能与医疗保健中干细胞递送的改进

Artificial intelligence (AI) is critical for improving the quality of stem cell manufacturing and delivery. AI can assist in determining the viability, effectiveness, efficacy, and safety of stem cells. Furthermore, in stem cell and regenerative medicine, AI is utilized to streamline simulation and model-building processes and find connections between cellular activities and their microenvironments. However, thoughtful consideration is required to minimize unwanted implications of AI incorporation for stem cell-based treatment.

The impact of the early stages of the COVID-19 pandemic on academic studies, clinical training, and their opinion on vaccination among radiography students 新冠肺炎大流行早期阶段对放射科学生学术研究、临床培训的影响及其对疫苗接种的看法

Objectives: Since its inception, the COVID-19 pandemic has generated challenges for healthcare professions and educational institutions worldwide. This study aimed to investigate the effect of COVID-19 on radiography students’ education in Jordan.Method: A cross-sectional online survey was conducted during the initial COVID-19 lockdown period (31 March to 30 May 2020). All diagnostic radiography year groups at governmental universities (the Hashemite University, Al-Hussein Bin Talal University, and Jordan University of Science and Technology) were invited to complete the anonymous survey in Jordanian using Microsoft Forms.Results: There were 417 responses. Findings showed that Jordan’s radiography students’ education has been significantly impacted by the COVID-19 epidemic. The majority of survey participants were not convinced the online learning platforms were user-friendly. Comparing remote teaching versus face-to-face instruction, 51% of participants had difficulty concentrating. Access to clinical placements was the biggest issue raised; 86% of respondents stated that their clinical experience differed from what was outlined in the curriculum. 42% of the students were in favor of receiving the COVID-19 vaccines.Conclusions: Clinical radiography education in Jordan has been significantly impacted by the COVID-19 pandemic. The availability of necessary facilities for online training, the efficiency of the online platforms, and motivation to learn were the significant issues. The adoption of blended learning strategies and the use of simulation to enhance practical placement chances need to be taken into consideration when developing future training curricula and responses to pandemics.

Hearing impairment in relation to vitamin D status in type 2 diabetic patients: A cross-sectional study 听力损害与2型糖尿病患者维生素D状况的关系:一项横断面研究

Objectives: To evaluate the association of serum 25 hydroxy vitamin D (25OHD) and hearing impairment type 2 diabetes mellitus (T2DM) patients. In addition, we aimed to examine whether T2DM-associated hearing impairment is correlated with the severity of diabetes.Methods: The present case-control study recruited adult patients with T2DM and healthy controls. Using pure-tone stimuli, we used an audiometer to assess the hearing thresholds of air conduction in each ear separately.Results: 84 patients and 32 volunteers were included. The serum 25OHD was lower in T2DM patients than in healthy control (12.24±1.3 versus 23.19±5.69, respectively; p<0.001). The prevalence of vitamin D deficiency was 100% in the T2DM group, compared to 25% in the control group (p=0.001). T2DM patients exhibited higher hearing thresholds at all tested frequencies compared to the control group (p<0.001). Diabetic patients with hearing impairment had a significantly lower serum 25OHD than the normal hearing group (11.7±1.2 versus 12.6±1.2 ng/d; p=0.02). The multivariate analysis demonstrated that serum 25OHD level was an independent predictor of hearing loss among diabetic patients (β=-0.605; p=0.041).Conclusions: the present study highlights the potential role of vitamin D deficiency in developing hearing impairment in the setting of T2DM. We found a negative correlation between serum 25OHD level and air conduction thresholds at low-mid and high frequencies amongst patients with T2DM.

Exosomes in cancer therapy: Advances and current challenges 癌症治疗中的外泌体:进展和当前挑战

Exosomes are subpopulations of extracellular vesicles (EVs) secreted by cells in normal or pathological conditions. From an oncological point of view, such vesicles are involved in cancer progression and may function as a therapeutic tool (based on EVs source), whose molecular signature plays a significant role in the investigation of cancer biomarkers. Recent research reveals the potential role of exosomes in anticancer drug delivery. In this article, we briefly discuss current theranostics perspectives on exosomes and their future orientation.

Continuity of care in acromegaly: Detecting and managing recurrence after surgical intervention 肢端肥大症护理的连续性:手术干预后复发的检测和管理

Acromegaly is a rare disease being followed up by primary care team. It is usually caused by pituitary adenoma and the treatment would be resection of the tumour. But is surgical intervention the end of treatment for acromegalic patient? This case highlighted the continuity of care in a 65-year old gentleman who had been diagnosed acromegaly since the age of 31. He had undergone a transseptal transsphenoidal resection of his pituitary adenoma when he was 45-year-old. Postoperatively, there were still residual growth hormone (GH) and insulin like growth factor (IGF-1) secretions. However, during 20 years follow up after operation, despite the increasing level of GH and IGF-1, the management of acromegaly were neglected as the focus of treatment were shifted towards his other medical problem, which is diabetes and atrial fibrillation. During his latest follow up, his complaint of unspecified headache and expressed his worry regarding the recurrence of his condition. Repeated GH level showed an increase in its level and postulated the possibility of recurrent acromegaly. This case emphasizes the important of recognizing the level of GH and IGF-1 after surgical intervention in order to detect recurrence acromegaly by primary care and early referral to endocrine team.

It is usually caused by pituitary adenoma and the treatment would be resection of the tumour. But is surgical intervention the end of treatment for acromegalic patient? This case highlighted the continuity of care in a 65-year old gentleman who had been diagnosed acromegaly since the age of 31. He had undergone a transseptal transsphenoidal resection of his pituitary adenoma when he was 45-year-old. Postoperatively, there were still residual growth hormone (GH) and insulin like growth factor (IGF-1) secretions. However, during 20 years follow up after operation, despite the increasing level of GH and IGF-1, the management of acromegaly were neglected as the focus of treatment were shifted towards his other medical problem, which is diabetes and atrial fibrillation. During his latest follow up, his complaint of unspecified headache and expressed his worry regarding the recurrence of his condition. Repeated GH level showed an increase in its level and postulated the possibility of recurrent acromegaly. Background: Stress urinary incontinence (SUI) is a widespread illness that mostly affects women, particularly those who have recently given birth or gone through menopause. The purpose of this meta-analysis is to compare the effectiveness of pelvic floor muscle exercises (PFME) plus biofeedback to PFME alone in treating SUI in female patients.Methods: We systemically searched six electronic databases (PubMed, Scopus, and Web of Science) from inception until February 7, 2022. We included randomized controlled trials (RTCs) comparing patients who had undergone PFME plus biofeedback to PFME alone. For risk of bias-2 (RoB2) assessment, we used cochrane risk of bias assessment tool. Continuous data were pooled as standardized mean difference (SMD), and dichotomous data were pooled as odds ratio with the corresponding 95% confidence intervals (CI).Results: 15 RCTs were included, with a total of 788 patients with SUI. The overall effect estimate between PFME+BF and PFME alone groups favored the PFME+BF group in improving PFME strength (SMD=0.33, 95% CI [0.14 to 0.52], p=.0009) and did not favor either of the two groups for quality of life (SMD=-0.22, 95% CI [-0.44 to 0.00], p=0.05), leakage (SMD=-0.10, 95% CI [-0.37 to 0.17], p=0.47), pad weight test (SMD=-0.22, 95% CI [-0.44 to 0.00], p=0.05), cure rate (odd ratio [OR]=2.44, 95% CI [0.52 to 11.42, p=0.26), and social activity (SMD=0.66, 95% CI [-0.04 to 1.36], p=0.07).Conclusion: BF addition to PRME improves cure rate and PFME strength without affecting leakage or quality of life. Healthcare providers must consider patient safety and comfort while choosing BF devices with PFME. SUI management strategies should include BF to improve results.

Factors associated with poor health-related quality of life among patients with asthma: A hospital-based study from Jordan 哮喘患者健康相关生活质量差的相关因素:约旦一项基于医院的研究

Purpose: Exploring the factors that are associated with diminished health-related quality of life (HRQOL) in patients with asthma is essential to reach the optimal HRQOL. This study aim to assess HRQOL and its associated factors in patients with asthma in Jordan.Methods: This cross-sectional study was conducted on patients with asthma attending King Abdullah University Hospital and Jordan University Hospital in Jordan. In addition to socio-demographic variables, the study survey included mini asthma quality of life questionnaire (mini AQLQ) to measure HRQOL and asthma control test (ACT) to assess the degree of asthma control in asthmatic patients. Binary logistic regression was used to build a model of the independent predictors of poor HRQOL in the study participants.Results: Of the 314 participating patients, 70.1% were females, with a mean age of 51.47±16.37 years. 61.1% of the participants had low HRQOL, with a total mean mini AQLQ score of 4.30±1.30. Results revealed that advanced age (OR=0.961; 95% CI: 0.936-0.986; p<0.05) and lack of daily exercise (OR=0.233; 95% CI: 0.085-0.637; p<0.01) were significant predictors of poor HRQOL in patients with asthma, while better controlled disease, manifested by higher ACT score was associated with better HRQOL in these patients (OR=1.499; 95% CI: 1.344-1.672; p<0.01).Conclusions: HRQOL represents an area for improvement in patients with asthma. The current study provides insight on the factors associated with poor HRQOL. Future interventions and asthma management programs should focus on improving asthma control and encouraging physical activity, particularly for older patients, with the aim of improving HRQOL among patients with asthma.

Role of ankle dorsiflexion in sports performance and injury risk: A narrative review 踝关节背屈在运动表现和损伤风险中的作用:一个叙述性的回顾

The objective of this literature review is to understand the role of ankle dorsiflexion range of motion in sports performance and the risk of injuries. The ankle harmonizes the interaction between the body and the supporting surface through adjusting to the supporting surface and handling forces to contribute effectually to different functional activities. Ankle dorsiflexion is an essential construct in many sport-specific skills. Ankle dorsiflexion is associated with activation of brain areas involved in movement preparation, sensory integration, motor planning/execution, balance, and visuomotor coordination. Ankle dorsiflexion was associated with enhanced activation of deep core and quadriceps muscles. Decreased ankle dorsiflexion is linked to compensations and altered kinetics and kinematics that can potentially affect sports performance and increase the chances of sustaining injuries. It is vindicated to consider more focus on ankle dorsiflexion range of motion in research studies, sports-related pre-season screening, clinical examination, injury rehabilitation, and return-to-sports judgment.

Objectives: The present study examines the importance of pain assessment, documentation, and education among nurses caring for critically ill patients.Methods: This cross-sectional descriptive study was conducted on a convenience sample of 200 nurses working in acute care settings of hospitals in Irbid, Jordan. Data were gathered via self-administered questionnaires. Eight items of the pain aassessment and management for critically ill questionnaire were used in this study to measure the importance of pain assessment, documentation, and education for nurses caring for critically ill patients.Results: There was a significant positive correlation between the use of pain assessment tools for patients’ able self-report and the importance of assessment tools. The nurses rated pain assessment in intensive care units patients with burns as being the most extremely important. Regarding the importance of assessing the need for pre-emptive analgesia before specific procedures, the most often rated as extremely important to do was the invasive line placement procedure. Most participants used the guidelines of clinical practice recommendations of the American Society of Pain Management Nursing for the assessment and management of pain.Conclusions: Poor pain assessment, documentation, and education are significant problems demanding a constructive re-appraisal of the present pain practices. Improving pain assessment, documentation, and education via extensive educational programs and campaigns is essential for optimal pain practice.
