Confirmed SARS-CoV-2 infection and mortality: Associated factors in hospitalized people 75 and older 确诊的严重急性呼吸系统综合征冠状病毒2型感染和死亡率:75岁及以上住院患者的相关因素

Introduction: COVID-19 infection in the elderly posed challenges in health systems and clinical care by health personnel.Objective: To describe the factors associated with mortality in persons aged 75 and older with COVID-19 in a high complexity hospital in Bogotá, Colombia.Methods: Observational, analytical and retrospective study, including 509 patients aged 75 and older hospitalized with COVID-19.Results: 40.47% died during hospital stay. It was found that a shorter time of symptom onset at admission, a respiratory rate greater than 20 breaths per minute, having thrombocytopenia, elevated lactate dehydrogenase and elevated D-dimer were associated with higher in-hospital mortality.Conclusions: There is an association between mortality and the presence of dyspnea, fever and delirium. Paraclinical results with lactate dehydrogenase >350 (U/L), the presence of elevated D-dimer greater than 1,000 μg/L, as well as a Pa02/Fi02 ratio with a median of less than 90, were associated with higher mortality.

Endocannabinoid system: An untold story in hypertensive nephropathy 内源性大麻素系统:高血压肾病不为人知的故事

Prognosis of hypertension leads to organ damage by causing nephropathy, stroke, retinopathy and cardiomegaly. Kidney, retinopathy and blood pressure (BP) have been discussed in plenty in relation with catecholamines of autonomic nervous system (ANS) and angiotensin II of renin angiotensin aldosterone system but very little have been told about the role of endocannabinoid system (ECS) in the regulation of kidney function, retinopathy and BP. ECS is a unique system in the body, which can be considered as master regulator of body functions. It encompasses endogenous production of its cannabinoids, its degrading enzymes and functional receptors, which innervate and perform various functions in different organs of the body. Kidney, retinopathy and BP pathologies arise normally due to elevated catecholamine and ang II, which are vasoconstrictor in their biological nature. Question arise which system or agent counterbalances the vasoconstrictors effect of noradrenaline and ang II in normal individuals? This review will not only try to illustrate the significance of ECS in the kidney and BP regulation but also establish the connection of ECS with ANS and ang II. This review will also explain that ECS, which is vasodilator in its action either independently counteract the effect produced with the vasoconstriction of ANS and ang II or by blocking some of the common pathways shared by ECS, ANS, and ang II in the regulation of kidney and BP regulation. This article conclude that persistent control of BP and normal functions of kidney is maintained either by decreasing systemic catecholamine, ang II or by up regulation of ECS, which will result in the regression of nephropathy, stroke, retinopathy, and cardiomegaly induced by hypertension.

Factors affecting job stress among physical therapists during the COVID-19 pandemic in Saudi Arabia 沙特阿拉伯2019冠状病毒病大流行期间影响物理治疗师工作压力的因素

Background: Physical therapists (PTs), like other healthcare professionals, are susceptible to job stress when involved in patient care, and several factors contribute to it. Therefore, this study aims to assess PTs’ knowledge, attitude, and practices (KAP) adopted by PTs during the COVID-19 pandemic and its impact on their job stress.Methods: A cross-sectional design was used. 300 PTs from the government and private healthcare organizations in Saudi Arabia were randomly picked and invited to participate. Data were collected using a pre-tested content-validated KAP tool using a Google Form between January and March 2022. Data were analyzed quantitively through SPSS 24.0.Results: The findings revealed that 85% of PTs whose job involves direct contact with the patients are susceptible to higher stress (perceived stress score [PSS]=22.82) than others. Specifically, female and unmarried PTs are prone to more job stress than others (p<0.05). In addition, PTs who were apprehensive about managing COVID-19 patients and worried about acquiring COVID-19 disease are prone to more job stress. Lastly, 89% of PTs who adopted the practice of wearing N-95 masks and proper handwashing techniques (>80%) experience less stress than others (p<0.05).Conclusion: The KAP of PTs influences job stress during the COVID-19 pandemic. PTs with sufficient knowledge about measures to adopt, show a positive attitude, and practice wearing proper PPEs, handwashing procedures, and waste disposal systems during patient care are prone to less job stress than others.

Exploring the role of knowledge brokering in developing specialized hospitals: A descriptive qualitative study 探索知识中介在专科医院发展中的作用:描述性定性研究

Background: Employing a knowledge broker (KB) is one way to transform ideas into action, especially in relation to the development of a specialized hospital. This study aimed to explore the concept of knowledge brokering intervention in the development of a specialize hospital in Indonesia.Methods: A descriptive qualitative method was applied in this study to collect detailed, rich data from a small number of participants. A snowball sampling approach was chosen to recruit 17 participants from various backgrounds who have worked with KB of a specialized hospital. The data analysis was performed using a content analysis method to identify the themes and categories.Results: Five themes and 14 categories were constructed for developing the knowledge brokering intervention in the development of a specialized hospital in Indonesia. The themes included characteristics of KB, the role of KB, demand brokering, various perceptions of specialized hospitals, and challenges in developing the specialized hospital.Conclusion: The findings suggest that the framework of the knowledge brokering intervention consisted of individual and interpersonal capabilities. Characteristics of KBs should be introduced that are trained in all development settings to boost the achievement of the goals in the health sector. The researchers suggest that the concept of KB should be considered for realizing national projects, as this will escalate the health development of the country. This study contributes to the identification and connection of indicators on health knowledge brokering in translating science into practice at specialized hospital in Indonesia.

Gene expression profile and presentation of novel gene variants of COX7B and COX7C in a cohort of patients with chronic myeloid leukemia 慢性髓性白血病患者队列中COX7B和COX7C基因表达谱和新基因变异的呈现

Chronic myeloid leukemia (CML) is a common blood malignancy awaiting decisive cure. Understanding the genomic nature of the disease could help to improve treatment strategies and introduce novel therapeutic targets. The purpose of this study was to sequence the entire transcriptome of a cohort of CML patients to understand differences in gene expression profiles and gene variants compared to healthy controls. RNA was extracted from 10 CML and four healthy control subjects and sequenced by the DNBSEQ platform. Differential gene expression was evaluated and confirmed by the q-RT-PCR technique. Gene variants were also analyzed. The results showed that COX7B and COX7C were upregulated in CML patients than in controls; this was confirmed by quantitative PCR. In addition, novel single nucleotide and insertion/deletion variants were also found in both genes. In conclusion, the results of this study recommend further exploration of COX7B and COX7C and their novel variants in myeloid leukemogenesis.

The anatomist’s perspective today towards human body donation for procurement of cadavers for study of human anatomy 解剖学家今天对人体捐赠用于人体解剖研究的尸体的看法

Anatomy is the study of structure of human body. Dissection of human cadavers has always been an integral part of study of anatomy as well as research in it. Till now the only source for cadavers for dissection purposes were unclaimed bodies. Today the need for bodies for medical education has increased and most medical schools are experiencing difficulties in procuring the requisite number of cadavers. Body donation programs may help fulfil this need of the medical schools. The present study was an attempt to analyze the attitude of the anatomists towards different aspects of body donation. 24.7% of the participants stated that they have insufficient cadavers, 12.3% claimed surplus cadavers while 62.8% claimed sufficient cadavers. 26% of the anatomists were unaware of the necessity for a death certificate and 35% were unaware of the necessity for a unique identification document for carrying out body donation.

Body donation programs may help fulfil this need of the medical schools. Introduction: Patients with diabetes are more likely to fall due to increased plantar pressure and decreased strength in the lower extremities.Objectives: To determine the influence of isometric ankle strength on dynamic foot plantar pressure in diabetes and non-diabetes participants.Methods: Twenty diabetes patients and twenty non-diabetes participants with age 28-54 years, height 150-182 cm, weight 48-90 kg, and BMI 25-54 kg/m2 participated in the study. The diabetes level was determined based on fasting plasma glucose levels. The resisted isometric muscle strength of the foot during dorsiflexion, plantar flexion, inversion, and eversion was measured using an electronic handheld dynamometer. The plantar pressure distribution during dynamic conditions was determined by using a 48.7×44.7 cm pressure platform. The outcome measures between diabetes and non-diabetes groups were statistically compared by student t-test. The correlation coefficient was determined by the Pearson correlation coefficient test. A p-value of less than 0.05 was considered significant.Result: The significant differences were found between diabetes and non-diabetes participants for the dorsiflexion (p=.048), plantarflexion (p=.031), inversion (p=.011), eversion (p=.024), peak pressure (p=.024), pressure per square inch (p=.012), pressure time integral (p=.014), and peak pressure gradient (p=.009). Significant relationships between resisted isometric ankle joint strength and foot plantar pressure for diabetes patients and non-diabetes participants were found.Conclusion: The present study’s findings reflect the higher frequency of plantar pressure distribution and higher muscle weakness in diabetes patients than in non-diabetes participants. These findings suggested that pressure data could help us to customize therapy strategies for patients with diabetes and prescribe a proper exercise intervention’s short-and long-term effects on gait biomechanics.

The model of screening for preeclampsia in the second and third trimesters of gestation 妊娠中晚期子痫前期筛查模型

Purpose: Preeclampsia (PE) is a specific syndrome of multiple organ insufficiency in case of pregnancy, which is included in the panel of major obstetric syndromes and is among the main causes of maternal morbidity and mortality in the whole world.Material and methods: We conducted a prospective cohort study of 91 pregnant women to evaluate the effectiveness of integrated use of maternal risk factors (2019 International Federation of Gynecology and Obstetrics recommendations), placenta location (ultrasound at 18-20 weeks of gestation), and serum cystatin C (at 18-36 weeks of gestation) in screening for pe in the second and third trimesters of gestation.Results: In the subgroup of pregnant women with cystatin C levels greater than 1.0 mg/L (27 women), PE developed in 26 women, which is 96.29% in percentage terms. When calculating GFR for cystatin C in a group with PE there was a significant violation of the renal filtration system -52.46±2.08 (95% CI, 48.39-56.54), while in healthy group the indicator is within normal limits -97.6±1.64 (95% CI, 94.38-100.82). In the analysis of the ratio of cystatin C levels more than 1.0 mg / l and the development of PE, a sensitivity of 98.46%, specificity of 100% and accuracy of 98.9%, p<0.001.Conclusions: The data show that the combined model of maternal factors, ultrasound of the placenta and serum cystatin C, is prognostically effective in pregnant women in the second and third trimesters of gestation and is a reliable marker for the development of pe.

A patient with toxic encephalopathy associated with acute carbon monoxide poisoning–A clinical case 急性一氧化碳中毒合并中毒性脑病1例临床分析

Carbon monoxide (CO) is a colorless, odorless, and lacking in irritative properties gas. It has a higher affinity towards hemoglobin than oxygen and thus the complex carboxyhemoglobin develops. Acute CO poisoning leads to poly-organic insufficiency and the organs that suffer the most are the ones with higher oxygen needs- the brain, the heart and the kidneys. We present a clinical case of 49-year-old woman with a sudden appearance of complaints of dizziness, stiffness throughout the entire body, inability to move. Subsequently she was found by her relatives with disorientated and “foaming around her mouth”. Earlier she had been using a gasoline generator because of a power outage. Her laboratory results came out with higher carboxyhemoglobin (40%), nitrogen waste products and mixed type acidosis. From the conducted brain MRI there were changes, correlating to toxic encephalopathy after acute CO poisoning.

This case illustrates the rare cause of failure to thrive (FTT) that initially presented with recurrent ear discharge. A five-year-old boy with a history of recurrent ear infections for the past year was treated for acute symptoms during each visit. He later was diagnosed with acute mastoiditis secondary to otitis media by a private otorhinolaryngologist and was referred to a tertiary hospital for admission and parenteral antibiotic commencement. The anthropometric evaluation noted he fell under the group of FTT and had an incidental finding of lower tract respiratory infection that turned out to be tuberculosis infection. Multiple complications occurred during his admission, including candidemia, disseminated tuberculosis, and deep-seated collections. Hence, he was worked up for primary immunodeficiency and was given extensive supportive treatment.
