Screening Oropharyngeal Dysphagia in Older Adults 筛查老年人口咽吞咽困难

This study investigated the use of the Eating Assessment Tool 10 (EAT-10) in predicting clinical outcomes in older adults hospitalized with COVID-19 infections between February and June 2021. The EAT-10 was performed for all patients. Thirty one percent and 23% of the 153 patients had oropharyngeal dysphagia risk and in-hospital mortality, respectively. Older age (hazard ratio: 1.08; 95% confidence interval, 1.03-1.13; P = .003) and higher EAT-10 score (hazard ratio: 1.02; 95% confidence interval, 1.01-1.04; P = .043) were associated with in-hospital mortality. Older age and having increased risk of oropharyngeal dysphagia were independently associated with a higher risk of in-hospital mortality in older patients with COVID-19.

Testing a Prioritization Tool to Identify Older Adults in Need of Home-Delivered Meals and Nutrition-Related Services 测试优先排序工具,识别需要家庭送餐和营养相关服务的老年人

The Expanded Food Security Screener (FSS-Exp) determines food insecurity risk based on physical, economic, and social access to food. It categorizes individuals into 5 priorities based on need for a home-delivered meal. Concurrent and construct validity were applied to the tool. For concurrent validity, partial correlations between FSS-Exp and health- and food-related measures were significant and ranged between 0.20 and 0.33. For construct validity, analysis of variance was used to examine scores of the health- and food-related tools by priority level. The 2 highest priority levels had significantly lower mean scores for all the health variables except for dietary intake. The FSS-Exp tool effectively identifies those most in need of meal services.

Impact of Preoperative Skeletal Muscle Mass on the Outcomes and Recurrence After Total Gastrectomy in Patients With Gastric Cancer 术前骨骼肌质量对胃癌患者全胃切除术后疗效和复发的影响

Low muscle mass is a predictor of poor outcome in patients with gastric cancer after surgery. Seventy patients who underwent gastric cancer resection with curative intent were included in the study. A cross-sectional area of muscle at the third lumbar vertebra was measured and patients with skeletal muscle index below 48.6 kg/m2 were described as having low muscle mass. Surgical and oncologic outcomes were recorded. Of 70 patients, 19 (27%) were diagnosed as having low muscle mass. The postoperative recurrence was significantly higher in patients with low muscle mass (31.6 %) than in those with no low muscle mass (11.8%). Preoperative low skeletal muscle is related to poor survival and recurrence in patients with gastric cancer after total radical gastrectomy.

Association of Weight Indicators, Dietary Habits, and Physical Activity With Common Benign Breast Diseases 体重指标、饮食习惯和体育锻炼与常见良性乳腺疾病的关系

Benign breast diseases (BBDs) are 10 times more common than breast cancer, and fibroadenoma (FA) and fibrocystic disease (FCD) are the most frequent. The association of BBD with physical activity (PA), weight indicators, and diet was investigated. Eligible women were entered in the FA, non-FA, FCD, and non-FCD groups. A PA and a food questionnaire were completed and anthropometric measures were taken and 779 women were studied. There was no significant difference regarding weight indicators and PA between the groups. Contrary to the general assumption, chocolate, tea, and coffee do not increase BBD, while chicken may elevate the risk.

Body Composition Analyses in Older Adults May Vary Significantly Depending on the Bioelectrical Impedance Analyzer Modality 老年人的身体成分分析可能因生物电阻抗分析仪模式的不同而存在显著差异

This study aimed to investigate the effects of aging on body composition analyses detected by a single-frequency (SF-BIA) and a multifrequency bioelectrical impedance analyzer (MF-BIA). Forty older patients and 42 young subjects were included in the study. Body composition was measured using an SF-BIA and an MF-BIA consecutively. Significant differences were found between the analyses of SF-BIA and MF-BIA in older patients. Handgrip strength values were found to correlate more strongly with the skeletal muscle mass index detected by MF-BIA. MF-BIA may be preferable to SF-BIA in older adults due to its higher correlation coefficient with handgrip strength.

Mobile Apps for Hypertension Management 高血压管理移动应用程序

Mobile applications (apps) show promise for managing hypertension; however, more evidence is needed to understand how dietitians use apps in hypertension care. Dietitians (n = 191) completed an online survey assessing app use in hypertension care. Most dietitians agreed that apps support behavior change, diet tracking, and goal setting. Apps are recommended for diet tracking more often than online programs or websites (P < .001), but not more than pen and paper. Dietitians generally recommended apps to some or most of their patients (40% and 25%, respectively); however, the majority (64%) do not recommend apps specific to hypertension (64%). Future work should examine barriers to app use for patients with hypertension.

The main aim of this study was to develop a smartphone application specializing in assessing dietary fiber intake of university students. The validity of the application was tested among 45 volunteers, their daily intake was recorded for 3 days using the Daily Fiber application, and intakes were compared with a 3-day food record matched to the same days. The mean intake of dietary fiber using the 3-day food record and our application was 10.08 ± 4.48 and 10.21 ± 4.64 g/d, respectively (P < .01). Overall, Daily Fiber is a promising tool for assessing dietary fiber intake among undergraduates.

The Effect of β-Hydroxy-β-Methylbutyrate on Muscle Strength and Functional Outcomes in Older Adults β-羟基-β-甲基丁酸对老年人肌肉力量和功能结果的影响

Nutritional intervention is increasingly being recognized as playing an important role in functional status. The supplementation with β‑hydroxy-β-methylbutyrate (HMB), which is a metabolite of an essential amino acid, may be an effective nutritional intervention to improve muscle strength and functionality in older adults. The aim of this narrative review was to present an updated overview of the effects of HMB supplementation in older adults, specifically on muscle strength and functional outcomes. A PubMed database search was performed and only randomized controlled trial studies were included. More than half of the 14 analyzed studies reported that HMB supplementation improved muscle strength and other functional outcomes. HMB supplementation has demonstrated some promising effects on muscle strength and functional outcomes in older adults with different conditions of health and physical activity.

The supplementation with β‑hydroxy-β-methylbutyrate (HMB), which is a metabolite of an essential amino acid, may be an effective nutritional intervention to improve muscle strength and functionality in older adults. The aim of this narrative review was to present an updated overview of the effects of HMB supplementation in older adults, specifically on muscle strength and functional outcomes. A PubMed database search was performed and only randomized controlled trial studies were included. More than half of the 14 analyzed studies reported that HMB supplementation improved muscle strength and other functional outcomes. Nutrition Management of the Adult Patient With Necrotizing Fasciitis 坏死性筋膜炎成人患者的营养管理

Necrotizing fasciitis is an invasive, life-threatening infection of the tissues. Necrotizing fasciitis is not a commonly seen infection, making it a challenging infection for many clinicians to treat and manage due to a lack of familiarity with the disease process. Further, there is limited research and guidance available regarding nutrition care for patients recovering from necrotizing fasciitis. This article presents a case study and highlights how currently available guidelines can be critically applied by dietitians to provide appropriate nutrition care in a relatively unfamiliar patient population.

The Effect of Oxandrolone on Weight Loss and Muscle Mass in Acute Spinal Cord Injury Patients 奥昔诺龙对急性脊髓损伤患者体重减轻和肌肉质量的影响

To investigate the impact of oxandrolone on weight and lean body mass (LBM) loss after acute spinal cord injury (aSCI), this observational study compared patients who received oxandrolone with historical controls. Age, sex, injury level, admission and discharge anthropometrics, nutrition interventions, length of stay, liver function tests, sternocleidomastoid muscle area (SMA) indexed to height on admission, and follow-up computed tomography scans were collected. Twenty patients receiving oxandrolone and 20 controls were included. Oxandrolone patients lost less weight; however, no differences were observed in SMA. Oxandrolone reduced weight loss following aSCI, but larger studies are needed to assess its impact on LBM.
