La radiopharmacie est une activité de pharmacie hospitalière à risque soumise à une réglementation complexe. Adopter la culture de l’audit est un enjeu prioritaire pour sécuriser le circuit du médicament radiopharmaceutique. L’objectif de l’étude est de démontrer l’intérêt de l’audit croisé, hybride entre l’audit interne et l’audit tierce partie.
Méthode
Un outil d’évaluation de 125 items s’appliquant à l’ensemble de l’activité de radiopharmacie a été conçu. Deux radiopharmacies ont été auditées en interne puis un audit croisé a été organisé entre les deux unités.
Résultats
Pour l’une des unités, 12 éléments ont été cotés différemment lors des deux audits. Quatre défauts de conformité sur les dix n’ayant pas été observés lors de l’audit interne ont été qualifiés de critiques par le radiopharmacien auditeur. Pour la seconde radiopharmacie, 15 items ont été cotés différemment dont deux ont été qualifiés de critiques.
Conclusion
Une opinion peut influencer involontairement un raisonnement mais l’indépendance de l’auditeur lors des audits croisés est un gage d’objectivité. C’est une alternative aux audits internes devenus routiniers et moins performants et à la différence des audits tierce partie, l’auditeur connaît les contraintes et les préoccupations du terrain. L’intérêt de cette démarche dépasse la simple évaluation des non-conformités. L’audit croisé favorise le partage d’expérience et de savoir-faire entre professionnels du même métier. Il créé une collaboration dynamique entre établissements et contribue à l’amélioration individuelle et collective de la sécurisation du circuit du médicament radiopharmaceutique.

More than two-thirds of suicides occur during a major depressive episode. Acting out prevention measures and therapeutic options to manage the suicidal crisis are limited. The impulsive-aggressive dimensions are vulnerability factors associated with suicide in patients suffering from a characterized depressive episode: this can be a dimension involved in animals. Impulsive and aggressive rodent models can help analyze, at least in part, the neurobiology of suicide and the beneficial effects of treatments. Ketamine, a glutamatergic antagonist, by rapidly improving the symptoms of depressive episodes, would help reduce suicidal thoughts in the short term. Animal models share with humans impulsive and aggressive endophenotypes modulated by the serotonergic system (5-HTB receptor, MAO-A enzyme), neuroinflammation or the hypothalamic-pituitary-adrenal axis and stress. Significant effects of ketamine on these endophenotypes remain to be demonstrated.

Les antirétroviraux du virus de l’immunodéficience humaine sont dispensés aux patients ambulatoires en France soit par les pharmacies d’officine, soit par les pharmacies hospitalières via la rétrocession. Plus de 20 ans après la mise en place de ce double circuit, il paraît nécessaire de s’interroger sur l’efficience de ces deux modalités de dispensation, tant en termes de service rendu aux patients que de dépenses pour l’assurance maladie.
Méthodes
Les données de l’assurance maladie ont été utilisées afin de quantifier les dispensations d’antirétroviraux dans le circuit officinal et hospitalier. Une enquête a été réalisée auprès des patients pour connaître leur intérêt envers la dispensation hospitalière, selon leur provenance géographique. Le coût différentiel, du point de vue de l’assurance maladie, entre les deux circuits de dispensation a été calculé sur la base des quantités rétrocédées et des prix d’achat dans notre centre hospitalier universitaire en 2018.
Résultats
Plus de 80 % des quantités d’antirétroviraux est désormais dispensée par les pharmacies d’officine. Les arguments en faveur de la dispensation hospitalière des antirétroviraux avancés par les patients sont l’anonymat et de disponibilité permanente des médicaments. L’assurance maladie est amenée à rembourser un médicament à des tarifs différents selon le lieu de délivrance de 37€ par boîte en moyenne, en faveur de la rétrocession.

Cette étude présente un état des lieux sur le double circuit de dispensation des antirétroviraux. Souvent mal connu des patients et des professionnels, il s’avère complexe tant aux étapes de prescription, de dispensation et de remboursement. Objective
A simple, highly specific, accurate and fast method by smartphone-based digital imaging was developed for estimating lidocaine hydrochloride in pharmaceutical formulations.
Material and methods
To obtain the images, a Galaxy A03 Core smartphone and an image acquisition device developed in the laboratory were used to control the incident factors in reproducibility of the measurements. The processing of the images was carried out with the Color Grab application. Finally, the absorbance values were calculated using the RGB intensity values of blank, standard, and sample solutions. The proposed method was compared with spectroscopic and chromatographic methods.
Results
The reaction between copper and lidocaine hydrochloride was characterized, showing better results in an equimolar ratio and maintaining the pH of the solution above 11.5. The use of the device for the capture of digital images allowed to control those sensitive parameters for reproducibility so that the analytical measurements showed adequate precision and accuracy. Validation of the main parameters of the method showed compliance with acceptance criteria. The application of the method for the analysis of injectable samples achieved reliable results, which were statistically similar to other reference instrumental methods.
Conclusion
The proposed method presented figures of merit in relation to linearity, precision, selectivity, accuracy, and robustness; it was carried out by designing and manufacturing a device for capturing digital images on a smartphone, which were analyzed to obtain RGB intensity values. These data are finally used to calculate absorbance values of solutions. All these elements provide this work with innovative characteristics in the field of analysis for control of pharmaceutical formulations.

Nitrosamines (NAs) are molecules that include the nitroso functional group. In 2018, the US Food and Drug Administration (FDA) received its first report of NAs in pharmaceuticals. The fact that NA impurities are likely human carcinogens is relevant to these compounds. Furthermore, prolonged exposure to NA contaminants above safe limits may raise the risk of cancer. The goal of this article was to assess the amounts of six different NAs in Sartan group medicines purchased from formal pharmacies in Istanbul, Türkiye, using a validated LC-MS/MS assay. An LC-MS/MS-based analytical assay was undertaken. The separation was performed with a HR ODS 150 mm × 3.0 mm and 5-analytical columns, providing effective separation of major peaks from NA impurities. In mobile phase A, formic acid was 0.10% in water, while in mobile phase B, formic acid was 0.10% in methanol. The flow rate was 0.4 mL/minute, and the total runtime was 18 minutes with the gradient elution mode. The validation was conducted in line with ISO/IEC 17025 requirements. Up to 100 μg/L, linearity was determined using correlation coefficients (r2 > 0.995) for all NAs. The limit of quantification values for all NAs analyses were below 1.0 μg/L. The mean recovery value obtained during the spike experiment was 95.18%, demonstrating the accuracy of the procedure. In addition, the accuracy was shown by a certified reference analysis, which yielded relative standard deviation and relative error values of 1.82% and 3.34%, respectively. During the intermediate precision testing, bias and relative standard deviation were 0.96 and 2.87%, respectively. Of the 75 study samples involving Sartan group medical products, no nitrosamine impurities were detected, demonstrating that pharmaceutical companies have adequate medication safety precautions in place in accordance with FDA and European Medicines Agency (EMA) regulations published to prevent NA contaminants in human medicinal products.

The study focuses to validate and develop a precise, simple and accurate stability indicating RP-HPLC method for estimation simultaneously of and silodosin and mirabegron in synthetic mixture. The chromatographic separation was achieved by using Shimpack Solar C18 column (250 × 4.5 mm, 5 μm) with acetonitrile: 5 mM ammonium acetate in ratio of 90:10% v/v as a mobile phase at a constant flow rate of about 1.2 mL/min. The development and validation were carried out at detection wavelength of 229 nm. We developed a robust RP-HPLC method, validated for linearity, precision, accuracy, specificity, and system suitability. The method demonstrated excellent linearity with correlation coefficient value r2 was nearly 0.998 with linearity range 8–18 μg/mL for Silodosin and 24–54 μg/mL for mirabegron. LOD and LOQ were found to be lower; hence, the method is sensitive. Percentage recovery was obtained 99.97% and 99.99% for silodosin and mirabegron, respectively. In case of precision, robustness and repeatability, RSD was found to be less than 2. The validated and developed RP-HPLC method offers an efficient and practical approach for the simultaneous quantification of silodosin and mirabegron in pharmaceutical formulations, making it a valuable tool for quality control and pharmaceutical research.
L’objectif de cette étude est de développer et valider et à une méthode indicatrice de stabilité RP-HPLC pour l’estimation simultanée de la silodosin et du mirabegron dans un mélange synthétique. Introduction
Le bon usage des anti-infectieux représente un défi en établissement de santé.
Objectif
Décrire le profil détaillé de consommation des anti-infectieux en 2021–2022. Décrire le profil évolutif annuel de l’utilisation des anti-infectieux en jours de traitement/1000 jours-présence, « antibiotic spectrum index »/1000 jours-présence et selon la proportion de jours de traitement administrés par voie parentérale de 2005–2006 à 2021–2022.
Méthode
Étude descriptive et rétrospective. Tous les patients admis au sein de notre centre hospitalier mère-enfant entre le 1er avril 2005 et le 31 mars 2022 ont été inclus. L’étude porte sur la consommation de tous les anti-infectieux selon la classification AWaRe de l’organisation mondiale de la santé et des « antibiotic spectrum index ».
Résultats
Un total de 673,9 jours de traitement/1000 jours-présence a été calculé en 2021–2022, soit 550,8 pour les antibiotiques, 46,5 pour les antiviraux, 67,9 pour les antifongiques et 8,7 pour les autres. Un total de 2436 « antibiotic spectrum index »/1000 jours-présence a été calculé en 2021–2021. Selon la classification AWaRe, en 2021–2022, 52,1 % (287/551) des jours de traitement/1000 jours-présence se référaient au groupe « Access », 47,8 % (263/551) au groupe « Watch » et 0,2 % (1/551) au groupe « Reserve ».
Conclusion
Cette étude vise à explorer l’utilisation des anti-infectieux dans un centre hospitalier mère-enfant. Elle met en perspective une utilisation stable voire en légère diminution d’une sélection d’anti-infectieux et se compare avantageusement aux données publiées. L’antibiogouvernance repose notamment sur l’évaluation périodique de la consommation. Notre étude illustre différentes approches pour quantifier et décrire cet usage.

Objectiver l’impact des disparités de processus de production et contrôle analytique sur la qualité des préparations de nutrition parentérale (NP) réalisées en région Auvergne-Rhône-Alpes.
Méthodes
Cette étude a été réalisée au sein de quatre pharmacie à usage intérieur (PUI) de la région Auvergne-Rhône-Alpes. Pour objectiver l’impact des processus de production, chaque PUI a produit dix préparations de NP à partir d’une même prescription. Des contrôles analytiques (dosages du sodium, potassium et calcium) ont été réalisés sur l’ensemble des préparations. Pour objectiver l’impact des processus de contrôle, un lot de dix préparations a été produit à partir d’une même prescription. Des échantillons ont été adressés aux quatre PUI pour la réalisation de contrôles analytiques (dosages du sodium, potassium et calcium).
Résultats
Les mesures de biais relatifs de production montrent qu’il existe une différence significative entre les préparations des quatre centres concernant les teneurs en sodium et potassium. Chaque centre présente au moins un biais de production vis-à-vis d’un des trois électrolytes dosés. Concernant les contrôles analytiques, il existe une différence significative entre les quatre centres concernant les teneurs en sodium et potassium mesurées. Sauf pour le calcium, tous les centres renvoient des mesures conformes aux spécifications usuelles de ± 10 % à la valeur cible.
Conclusion
La diversité des pratiques de NP a un réel impact sur la qualité des préparations effectuées. Un travail collaboratif régional est à envisager pour homogénéiser la prise en charge des patients.
Objectives
To assess the impact of disparities in production and analytical control processes on the quality of parenteral nutrition (PN) preparations produced in the Auvergne-Rhône-Alpes region.
Methods
This study was carried out in four hospital pharmacies of the Auvergne-Rhône-Alpes region. To assess the impact of production processes, each centre produced ten PN preparations from the same prescription. Analytical controls (sodium, potassium and calcium dosage) were carried out on all the preparations. To assess the impact of the control processes, a batch of ten preparations was produced from the same prescription. Samples were sent to the four hospital pharmacies for analytical control (sodium, potassium and calcium dosage).
Results
Measurements of relative production bias show that there is a significant difference between the preparations from the four centres in terms of sodium and potassium content. Each centre had at least one production bias for one of the three electrolytes measured. Pour objectiver l’impact des processus de production, chaque PUI a produit dix préparations de NP à partir d’une même prescription. Des contrôles analytiques (dosages du sodium, potassium et calcium) ont été réalisés sur l’ensemble des préparations. Pour objectiver l’impact des processus de contrôle, un lot de dix préparations a été produit à partir d’une même prescription. Des échantillons ont été adressés aux quatre PUI pour la réalisation de contrôles analytiques (dosages du sodium, potassium et calcium).</p></div><div><h3>Résultats</h3><p>Les mesures de biais relatifs de production montrent qu’il existe une différence significative entre les préparations des quatre centres concernant les teneurs en sodium et potassium. Chaque centre présente au moins un biais de production vis-à-vis d’un des trois électrolytes dosés. Concernant les contrôles analytiques, il existe une différence significative entre les quatre centres concernant les teneurs en sodium et potassium mesurées. Sauf pour le calcium, tous les centres renvoient des mesures conformes aux spécifications usuelles de<!--> <!-->±<!--> <!-->10 % à la valeur cible.</p></div><div><h3>Conclusion</h3><p>La diversité des pratiques de NP a un réel impact sur la qualité des préparations effectuées. Un travail collaboratif régional est à envisager pour homogénéiser la prise en charge des patients.</p></div><div><h3>Objectives</h3><p>To assess the impact of disparities in production and analytical control processes on the quality of parenteral nutrition (PN) preparations produced in the Auvergne-Rhône-Alpes region.</p></div><div><h3>Methods</h3><p>This study was carried out in four hospital pharmacies of the Auvergne-Rhône-Alpes region. To assess the impact of production processes, each centre produced ten PN preparations from the same prescription. Analytical controls (sodium, potassium and calcium dosage) were carried out on all the preparations. To assess the impact of the control processes, a batch of ten preparations was produced from the same prescription. Samples were sent to the four hospital pharmacies for analytical control (sodium, potassium and calcium dosage).</p></div><div><h3>Results</h3><p>Measurements of relative production bias show that there is a significant difference between the preparations from the four centres in terms of sodium and potassium content. Each centre had at least one production bias for one of the three electrolytes measured. Concerning analytical controls, there was a significant difference between the four centres in the sodium and potassium levels measured.
