La chirurgie des personnes transidentitaires n’est pas appréhendée par la majorité des chirurgiens plasticiens comme une simple chirurgie à but de transformation morphologique. Parallèlement la population trans française ne bénéficie pas d’une couverture chirurgicale adéquate. Au cours des dernières années, la réglementation française s’est allégée sur le parcours des chirurgies de réassignement. De plus nous avons assisté à une progression assez rapide des demandes de chirurgie de transition avec des parcours accélérés et parfois atypiques. Depuis quelques années un certain nombre de spécialistes ont mis en garde la communauté médicale sur les risques de dérapage en raison d’un manque de suivi psychologique de certaines personnes entamant un parcours de transition. Il est également apparu assez récemment des parcours de transition hybrides qui ne se résument plus d’un point de vue chirurgical à faire en sorte qu’une patiente assignée comme femme à la naissance puisse revêtir une apparence la plus masculine possible ou l’inverse. Nous exposons dans ce manuscrit la place biologique, historique et sociétale de la transidentité puis abordons les raisons des mises en garde d’une certaine catégorie de la population médicale tout en rassurant la communauté chirurgicale sur le bienfondé des chirurgies de réassignements dans un contexte contrôlé. Nous terminons par proposer quelques voix d’amélioration du parcours des personnes transidentitaires applicables en France.
The surgery of trans people is not apprehended by most of plastic surgeons as a simple surgery for the purpose of morphological transformation. At the same time, the French trans population does not benefit from adequate surgical coverage. Over the past few years, French regulations have simplified the process of reassignment surgeries. In addition, we have witnessed a fairly rapid increase in requests for transition surgery with accelerated and sometimes atypical courses. In recent years, a number of specialists have warned the medical community about the risks of slippage due to a lack of psychological monitoring of certain people beginning a transition process. Quite recently, hybrid transition paths have also appeared which, from a surgical point of view, are no longer limited to ensuring that a native assigned female patient can take on the most masculine appearance possible or the reverse. In this manuscript, we expose the biological, historical and societal place of transidentity and then address the reasons for the warnings of a certain category of the medical population while reassuring the surgical community on the benefits of reassignment surgeries in a controlled context. We end by proposing a few ways to improve the care course of trans people applicable in France.

Objectif
L’objectif de cet article est de décrire de manière exhaustive la technique du lambeau péritonéal prélevé par voie cœlioscopique dans la chirurgie d’affirmation de genre homme-à-femme (HtF), en particulier dans les cas de défauts de profondeur vaginale post-vaginoplastie par lambeau pénien inversé.
Résultats
Nos résultats à court terme révèlent que la vaginoplastie par lambeau péritonéal, adaptée de la procédure de Davydov, montre un potentiel significatif pour l’amélioration des résultats fonctionnels et esthétiques, y compris la création d’un néo-vagin autolubrifiant. Cependant, la complexité de la procédure nécessite une expertise chirurgicale avancée et des soins postopératoires adaptés. La sélection des patientes joue également un rôle essentiel, car toutes les patientes ne sont pas des candidates idéales pour cette procédure.
Conclusion
Malgré ses promesses, l’adoption généralisée de la technique du lambeau péritonéal dans la chirurgie d’affirmation du genre HtF est entravée par plusieurs défis, notamment la nécessité d’une formation chirurgicale spécialisée et les complications postopératoires potentielles. Aim
The aim of this article is to provide a comprehensive description of the peritoneal flap technique in male-to-female (MtF) gender affirmation surgery, particularly in cases of insufficient depth after penile inversion vaginoplasty.
Results
Our short-term results reveal that the peritoneal flap vaginoplasty, adapted from the Davydov procedure, has shown significant potential for improving functional and aesthetic outcomes, including the creation of a self-lubricating neovagina. However, the complexity of the procedure requires advanced surgical expertise and appropriate postoperative care. Patient selection also plays an essential role as not all patients are ideal candidates for this procedure.
Conclusion
Despite its promises, the widespread adoption of the peritoneal flap technique in male-to-female (MtF) gender affirmation surgery is hindered by several challenges, including the need for specialized training and potential postoperative complications. Thus, this technique should be considered as an alternative or complement to traditional methods, depending on individual patient factors. La mucormycose est une infection fongique rare et grave, survenant majoritairement sur des terrains immunodéprimés, diabétiques, des polytraumatismes ou des brûlures. Les traitements de références actuels reposent sur des parages chirurgicaux carcinologiques itératifs, un traitement systémique par amphotéricine B liposomale et en deuxième ligne par du Posaconazole ou Isavuconazole. Le cas d’une patiente de 37 ans sans antécédents, ayant présenté une mucormycose disséminée, avec perte de substance cutanée estimée à 25 % et délabrement majeur de la paroi thoracique est rapporté. En plus du traitement de référence, des instillations locales d’amphotéricine B en utilisant le système VAC Veraflow® ont été réalisées. Nous pensons que les instillations locales d’amphotéricine B par VAC pourrait permettre d’améliorer le pronostic fonctionnel des patients présentant une atteinte cutanée.
Mucormycosis is a rare and serious fungal infection, occurring mainly in immunocompromised, diabetic, polytrauma or burn patients. Current standard treatments include iterative carcinological surgical trimming, systemic treatment with liposomal amphotericin B and second-line Posaconazole or Isavuconazole. We report the case of a 37-year-old female patient with no previous medical history who developed a disseminated mucormycosis, with an estimated 25 % loss of skin substance and major decay of the chest wall. In addition to standard treatment, local instillations of amphotericin B using the VAC Veraflow® system were performed. We believe that local instillations of amphotericin B by VAC could improve the functional prognosis of patients with skin involvement.

Background
The Deep Inferior Epigastric Perforator (DIEP) flap is a modality in breast reconstruction of choice. Despite its well-documented benefits and complications, a lack of evidence remains with regards to the risks of performing a bilateral versus a unilateral reconstruction. As such, we sought to compare the rates of adverse outcomes in the perioperative and postoperative periods associated with a unilateral versus a bilateral DIEP flap breast reconstruction.
Methods
A retrospective cohort study of 178 consecutive patients undergoing unilateral versus. bilateral deep inferior epigastric perforator flap breast reconstruction was performed at our tertiary care center over a 3-year period. Data on demographics, operative time, intraoperative and postoperative complications, and surgical re-exploration, were extracted for both groups. Statistical analysis was performed on a per-flap basis.
Results
A total of 157 unilateral and 42 bilateral deep inferior epigastric perforator flaps were identified. The rate of intra-operative complications was 12.1% for unilateral versus. 4.8% for bilateral flaps (P = 0.26). Total post-operative complications rates were 30.6% for unilateral versus 54.7% for bilateral flaps (P = 0.003). Surgical re-exploration was performed in 12.7% of unilateral and 11.9% of bilateral cases (P = 0.88). The rate of total flap loss was similar between types of reconstruction, occurring in 2.5% of unilateral vs. 2.4% of bilateral flaps (P = 1).
Conclusion
This study demonstrates the rate of complications per flap is significantly higher in bilateral versus unilateral deep inferior epigastric perforator flap breast reconstruction. Bilateral DIEP breast reconstruction should be decided on a case-by-case basis.

With the rise of risk prevention surgery, more and more bilateral mastectomies are performed. These present a challenge when patients desire autologous reconstruction. Some surgeons perform reconstructions using a double DIEP flap, but this technique, which requires rapid operation and extensive experience, can sometimes be difficult to implement. Other factors may also favor a two-stage reconstruction, one breast after the other. We present here our technique to harvest two hemi-DIEP during two distinct operating times, by modifying the design of the first flap and by performing a prior autonomization of the second.

The medial plantar flap is frequently used in heel reconstruction and has been described since 1969. We took care of a 25-year-old patient with a bilateral ballistic trauma and open fractures to the distal third of both legs. Given the seriousness of the damage to the left lower limb, a trans-tibial amputation had to be performed. Concerning the right lower limb, we decided to cover the defect with a spare part's free medial plantar flap thanks to the amputated fragment. The extremity spare tissues principle is already known and described, more particularly concerning hands, in emergency traumatic surgery and in planned surgery, mainly concerning thumb reconstruction, for example for hypoplasia or after traumatic amputation. Apart from an article on the use of foot spare part in the coverage of amputation stumps, no article has reported the use of a foot spare part in reconstructive surgery, even more in traumatic surgery. We believe that the use of an extremity spare tissues should be considered in rare cases where this is possible, in order to reduce donor-site morbidity in the context of reconstructive surgery.
Le lambeau plantaire interne est fréquemment utilisé dans la reconstruction du talon. Nous avons pris en charge un patient de 25 ans présentant un traumatisme balistique bilatéral et des fractures ouvertes du tiers distal des deux jambes. Compte tenu de la gravité des lésions du membre inférieur gauche, une amputation trans-tibiale a dû être réalisée. En ce qui concerne le membre inférieur droit, nous avons décidé de couvrir la perte de substance cutanée par un lambeau libre médial plantaire en « pied-banque » grâce au fragment amputé. Background
The purpose of this study is to investigate the usefulness of endoscopy-assisted latissimus dorsi muscle flap (LDMF) harvesting in immediate breast reconstruction following partial mastectomy.
Materials and methods
From November 2016 to December 2019, sixteen female breast cancer patients who underwent immediate breast reconstruction following partial mastectomy underwent LDMF harvesting with endoscopic assistance. This surgical technique was carried out with only one subaxillary skin incision without leaving a scar on the back. Patients’ demographic characteristics, histopathologic factors, operative data, postoperative complications, and oncologic safety were collected through electronic chart review.
Results
In sixteen patients, LDMFs were harvested successfully using an endoscopy-assisted technique without conversion to an open technique. The mean age of the patients was 48.0 ± 8.0 years, and the mean body mass index was 24.4 ± 3.9 kg/m2. The most common histologic subtype was invasive ductal carcinoma, with a mean tumor size of 3.2 ± 2.3 cm. In terms of LDMF harvesting time, it took 168.4 ± 44.0 minutes. The most common postoperative complication, donor site seroma (75%), was managed non-surgically during the outpatient visit. In terms of cosmetic aspects, we’ve seen a high level of patient satisfaction, especially with scarring.
Conclusions
Endoscopy-assisted LDMF harvesting technique is safe and useful for breast reconstruction after partial mastectomy. Compared to the conventional open technique, this method does not leave a long scar on the donor site. As a result, it leads to better cosmetic outcomes and improves patient satisfaction.

Spontaneous extensor digitorum communis (EDC) tendon rupture is uncommon but easily confusing injury that lead to functional impairment. There is no any strict first-line treatment for nonrheumatoid EDC tendon rupture. We report a kidney transplant recipient with spontaneous rupture of the lateral expansion of the long-finger extensor tendon treated by wide awake surgery. Surgical repair with wide-awake local anesthesia was performed to realign the tendon for the patient. Three months following surgery, the patient obtained a full pain-free range of motion of the hand without recurrent dislocation.
La rupture spontanée du tendon de l’extenseur digitorum communis (EDC) est rare, mais facilement déroutante, car elle entraîne une déficience fonctionnelle. Il n’existe pas de traitement strict de première intention pour la rupture du tendon EDC non rhumatoïde.
