Introduction: Orofacial myofunctional reeducation (OFMR) is the rehabilitation of the muscles, functions and resting postures of the orofacial complex. It is used in the therapeutic management of orofacial dysfunction in patients of all ages and with a wide range of disorders and comorbidities.

Objective: The main objective of this article was to advocate the association of orofacial myofunctional reeducation (OFMR) with the use of prefabricated reeducation appliances (PRAs) in orthodontics.

Material and method: A summary of published data was carried out and the conclusions of a cross-sectional epidemiological study were reported. The aim of the study was to assess the current state of orofacial myofunctional rehabilitation in orthodontics in France. Consideration was given to the relevance of the arguments put forward by manufacturers regarding the specific features of their devices and their supposed effects.

Results: Published data, albeit of uneven methodological quality, seem to show the superiority of orofacial myofunctional reeducation combined with the use of a prefabricated reeducation appliance (PRA), compared with the use of OFMR without PRA. A new, more effective, simpler and less time-consuming approach to PRA-assisted OFMR is proposed. The project for a new model of prefabricated reeducation appliance is presented. Ten clinical cases are presented to illustrate the clinical and administrative aspects of this PRA-assisted RMOF. Original clinical approaches are also presented: mandibular disocclusion and recentering wedges (MDRW), anti-retrusion wedges (ARW) and sequential multi-attachment bonding (SMB), mandibular then maxillary.

Conclusion: PRA-assisted OFMR appears to be a necessary paradigm shift, which it would be useful to offer our patients.

Introduction: Pr. Delaire has understood since 2005 that functional therapy shouldn't try to rehabilitate a praxis but instead focus on the whole created by different oro-facial functions to ensure the creation of a global dynamic balance. That's why rehabilitation of the right tongue posture is a necessary condition for automation as nasal breathing; in the same meaning that rehabilitation of nasal breathing is a necessary condition for automation of the right tongue posture. Fortunately, nature can help us if we respect and follow physiological processes that take into account key points of « connectionism ». As a reminder that « learning isn't only the ability to produce an action, a behaviour that was unknown before, it's a process that need to inhibate automatical mental processes ».

Materials and methods: The author will develop his philosophy of « connectionism », based on his personal research, his readings on neuroscience and oro-facial functions and his forty years of clinical practice.

Conclusion: Orofacial functions influence each other and inhibition of each dysfunction is necessary to automate the newly obtained myofunctional balance.

Introduction: The surgical orthodontic patient is a patient whose management is specific. The success of this protocol goes beyond of the right indication and the control of the pre-surgical orthodontic preparation of the treatment. The coordination revolves around a multidisciplinary team, namely an orthodontist, a maxillofacial surgeon, a physiotherapist, a speech therapist, a general practitioner, etc. but also with the collaboration of a specialized psychotherapist.

Material and method: Does the ideal « patient » exist in orthognathic surgery? The decision criteria are morphological (occlusal, aesthetic and cephalometric) but also psychosocial.

Results: A new classification called « psychological » is proposed in this article.

Conclusion: This new classification can be considered as a key decision in the « casting » of the surgical orthodontic patient and represents a first basis for further investigations in this field.

Introduction: The democratization of access to orthodontic treatments and the development of orthodontics in adults lead more and more practitioners to take care of patients with various general pathologies. The aim of this literature review was to specify the recommendations governing orthodontic treatment in six specific situations: diabetes, patient at risk of endocarditis, haemophilia, osteopetrosis, epidermolysis bullosa and treatment with growth hormone.

Material and method: A review of the literature was carried out according to the PRISMA method from the PubMed database. The keywords used were, for diabetes: diabetic, diabetus mellitus, for infective endocarditis: endocarditis, for hemophilia : hemophilia, bleeding disorder, for the use of growth hormone: growth hormone, for epidermolysis bullosa: epidermolysis bullosa and for osteopetrosis: osteopetrosis, associated with the request « orthodontic » or « tooth movement ».

Results and conclusion: None of these situations constituted an absolute contraindication to orthodontic treatment. However, if surgical procedures with potential for complications were necessary, a compromise should then be discussed in order to provide the best benefit/risk balance for patients.

Introduction: The diode laser, which appeared at the end of the last century, is becoming more and more important in the various dental specialties, more particularly in orthodontics where the first publications date back to 2004. Today it has become an indispensable instrument for the orthodontist, who can make his patients benefit from this technology and its essential contribution both in terms of ablative treatment and photobiomodulation.

Materials and methods: The article will describe all the current applications of the diode laser in orthodontics, including the new perspectives that it can generate.

Results: Through the bibliography, we were able to identify the main surgery and photobiomodulation actions according to the different pathologies and our desired orthodontic treatments. We have not developed the different protocols in an exhaustive way.

Conclusions: There are certainly still many applications of laser in our specialty that are not sufficiently developed or known.

Introduction: Like the majority of medical professions, odontology is a regulated profession whose exercise is subject to legal provisions.

Material and method: The bases of these regulatory obligations are detailed and analysed, in particular the obligations concerning the relationship with patients, their information and obtaining their consent prior to any treatment. The obligations of the practitioner himself are then specified.

Conclusion: Compliance with regulatory provisions is intended to provide a secure framework for practice and to promote a good patient-practitioner relationship.
