Objective: To analyze the management model adopted in Emergency Services in municipalities that are part of a Regional Health Department. Methods: Mixed methods study with a concomitantly incorporated design. It was conducted in 2017 with six managers and 223 health workers (physicians, nurses and nursing technicians/assistants) from ten Emergency Care Units in eight municipalities of the Regional Health Department XIII, São Paulo. The data collection instrument composed of six closed questions and four open questions was delivered to participants. Quantitative data were analyzed using descriptive statistics; qualitative data were developed using the Collective Subject Discourse technique and analyzed using the DSCSoft® software. Results: There was a predominance of female professionals, 173 (75.2%), in the position of nursing assistants, 78 (33.9%). Managers evaluated the institution’s management model as contemporary and participatory, enabling workers to participate in decision making and giving them autonomy to decide on day-to-day issues, while workers evaluated it as traditional management, highlighting its formal structure. Conclusion: It was observed that managers and workers have a different view of the management model of health institutions. The assumptions of the shared management model encompass valuable concepts, such as dialogic communication, decentralization of power and greater autonomy for decision making. However, when analyzing the opinion of most workers who participated in this study, the wide use of a hierarchical model in institutions is still observed.

Objectives: To identify how scholars define excessive screening for women without risk of developing breast cancer, examine the determinants (barriers and facilitators) of excessive use of mammography screening, and describe the rates of observations of excessive use mammography screening. Methods: Scoping review based on a search in May 2022 in six electronic health databases and libraries. Articles included were peer-reviewed articles, in any language and year of publication. Results: In a sample of 18 articles, published from 1991 onwards, most of them from the United States, the excessive use of mammography were defined as the intention or performance of mammography outside the recommended age or interval range, among women with limited life expectancy, in coexisting, organized and opportunistic programs. The rates of observations of excessive use of mammography screening in the selected studies ranged from 1.4% to 87,2%. Facilitators for excessive mammography are related concerns of getting cancer; to the medical advice, especially from specialists; and to the increased access to tests. The most exposed to excessive screening are women with higher levels of education and income. Barriers for excessive mammography included guidance in consultations about the harm of mammography and life expectancy by general practitioners, particularly those in primary care. Conclusion: Our study identified that the excessive use of mammography screening has a high prevalence when done as screening and is permeated by multi-level factors. Our list of determinants can provide some guidance for future studies aiming to de-implement the low-value care of excessive mammography screening.

Objective: To evaluate the prevalence of risk for Burnout Syndrome among health professionals in areas of care for patients with COVID-19, as well as to verify possible associations of the syndrome with the perceived social support and food insecurity of these workers. Methods: Analytical cross-sectional study, with workers from intensive care units (ICU) and wards caring for patients with COVID-19, in a university hospital. Data collection was carried out from September to October 2021, using the instruments: Maslach Burnout Inventory (MBI-HSS), Multidimensional Perceived Social Support Scale and Food Insecurity Experience Scale (FIES). Statistical analysis used Poisson regression models and multiple Poisson regression, considering statistically significant differences and associations if p<0.05. Results: A total of 75 workers from three wards (48%) and from one ICU (52%) participated in the survey, and the professionals are mostly female (89.3%), trained as health care technicians (66.7%). Regarding the risk of Burnout Syndrome, 26.7% of the workers had scores for at least one dimension of the scale, mainly referring to high emotional exhaustion (20%). There was a positive association between the risk of developing Burnout Syndrome and food insecurity (PR = 1.11; 95%CI = (1.04; 1.18); p = 0.002). The number of children was significantly negatively associated with the incidence of Burnout Syndrome (PR = 0.90; 95%CI = (0.83; 0.97); p = 0.008). Conclusion: Positive associations were observed with a higher risk of BS in professionals with food insecurity and also that the number of children acts as a protective factor against the risk of Burnout Syndrome, which may be directly related to perceived social support.

Objective: To identify, in the scientific literature, the barriers that make it difficult to apply the Safe Childbirth Checklist of the World Health Organization. Methods: An integrative review was conducted from November 2020 to May 2022, using the following data sources: Scopus, MEDLINE®/PubMed®, Web of Science, and CINAHL. This study was conducted according to the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses protocol. Results: The sample consisted of 14 studies published mainly in 2021, when South America predominated as a publishing continent. The English language, methodological studies, and quantitative approaches prevailed. Level IV evidence prevailed in the sample. In the identified studies, there was a strong description of cultural factors, followed by structural factors and factors related to the work process. Conclusion: Cultural (interpersonal relationships, hierarchy of professional classes, and poor communication) and structural (design and fonts used in the checklist) factors, and those related to the work process (such as the checklist implemented in the health service, the manager’s attitude regarding presenting it, and need for educational/training intervention for health professionals) are the main barriers that make it difficult to apply the Safe Childbirth Checklist.

Objective: To map the health care of Pegfilgrastim On-body Injector in neutropenia prevention in adults with cancer in home care after outpatient chemotherapy. Methods: This is a scoping review based on the JBI methodology. Only studies with adults with cancer undergoing outpatient chemotherapy were included. The search was carried out in the Cochrane, CINAHL, EMBASE, LILACS, PubMed, Scopus, LIVIVO and Web of Science databases, in addition to gray literature ProQuest, SciELO, Database in Nursing, Google Scholar, Open Grey, drug leaflet and websites. The searches in the references of selected studies were exhausted. All identified studies were exported to the EndNote reference manager for organization and removal of duplicates. The Rayyan web application was used for evidence selection. The studies were selected by pairs independently, with conflicts resolved by a third researcher. Results: A total of 10 articles were included, whose results were subdivided into categories: patient compliance, health team opinion, patient workload in cancer treatment and device use in clinical practice. The device has few flaws and was accepted by health care teams and patients in most studies. Conclusion: The main health care for Pegfilgrastim On-body Injector use is related to the skin preparation technique where the device will be applied, in addition to device preparation and administration. Moreover, the importance of assessing the knowledge of patients and their family about the device is highlighted, providing all the necessary guidelines, verbally and in writing, clearly and objectively, and validating this information, making sure that patients have understood all of them and are safe.
目的:探讨聚非格司汀在成人癌症患者门诊化疗后家庭护理中预防中性粒细胞减少的保健作用。方法:这是一个基于JBI方法的范围审查。仅纳入了接受门诊化疗的成年癌症患者的研究。检索Cochrane、CINAHL、EMBASE、LILACS、PubMed、Scopus、LIVIVO和Web of Science数据库,以及灰色文献ProQuest、SciELO、Database in Nursing、谷歌Scholar、Open Grey、药物手册和网站。

Objective: To evaluate patients’ and healthcare professionals’ knowledge about vaccination protocols in post-liver transplantation before and after applying an educational awareness strategy in this population. Methods: Patients (n=124) underwent the educational intervention through access to a webpage containing educational videos and, for health professionals (n=111), through a symposium and access to information on the project’s virtual page. To analyze the effect of the intervention, qualitative analyses of knowledge were carried out using questionnaires before and after the interventions. Results: Among patients, males were predominant (66.9%) and the mean age was 55.2 years old (SD + 15.9). 82.2% of patients visited a UBS to be vaccinated and 13.7% of them the CRIEs (Reference Center for Special Immunobiologicals). Only 46.7% received orientation about vaccines after liver transplantation. From the 111 questionnaires answered by health professionals, 46.5% did not check the vaccine portfolio, 61.3% referred patients to UBS and 38.7% to a CRIE. In the post-intervention analysis, 66.1% of patients watched educational videos about vaccination. Of these, 62.2% said they had improved their understanding about vaccines and 91.4% feel safer to vaccinate. After the educational intervention, 45 health professionals answered the questionnaire. 30.4% said they knew which vaccines to prescribe, and 67.4% recommended vaccines to patients’ relatives. Conclusion: The proposed educational strategy applied in this study shown to increase awareness regarding the post-liver transplant immunization protocols. This may contribute to avoiding the potential risk of lack of information and failure to address vaccination by healthcare professionals.

Objective: To carry out content validity of the statements of International Classification for Nursing Practice (ICNP®) terminology subset diagnoses, outcomes and nursing interventions for people with chronic kidney disease on conservative treatment. Methods: A methodological study, which followed the Brazilian method guidelines, with content validity conducted by the Delphi technique and carried out with 67 expert nurses. A total of 123 nursing diagnosis/outcome statements and 215 nursing intervention statements, distributed by Callista Roy’s adaptation model of nursing modes were assessed. To assess the degree of agreement regarding the meaning and clinical use among experts, the Content Validity Index (CVI) was used, accepting statements that presented an index ≥ 0.80. Results After two Delphi rounds, 117 diagnoses/outcomes and 199 nursing interventions were validated. Of the diagnoses, 70 (60%) were classified in Physiological Mode, 19 (16%) in Self-Concept Mode, 17 (14%) in Real-Life Function Mode, and 11 (10%) in Callista Roy’s Interdependence Mode. Among the most prevalent diagnostic statements were: “Altered Blood Pressure”, “Peripheral Oedema”, “Fluid Retention”, “Lack of Knowledge of Dietary Regime”, “Impaired Adaptation”, “Self-Care Deficit” and “Impaired Access to Treatment”. Valid nursing interventions underwent wording changes. Conclusion: The development of an ICNP® terminology subset according to the adopted theoretical model proved to be valid in terms of content for the care of people with chronic kidney disease undergoing conservative treatment through clear and directive care plans.
